Position Summary:
The Social Worker will support the delivery of instrumental and counselling Social Work Services to improve the quality of life for ONE CARE clients and their caregivers. The Social Worker completes client assessments and coordinates required services as well as providing counselling services. There is a strong emphasis on liaising with appropriate resources in the community to ensure clients and families are linked with the necessary supports. The incumbent demonstrates the ability to prioritize a diverse caseload of clients and respond to crisis situations as they arise.
This role is designated as 37.5 hours per week Monday to Friday, as well as an on call rotation, including evenings and weekends.
QUALIFICATIONS:
- Minimum qualifications include a Bachelor’s Degree in Social Work from a recognized educational institution. Preference provided to those with a Master’s degree.
- Current registration in good standing with the Ontario College of Social Workers. * Social Work Registrant Required.
- Demonstrated skill in assessment with the ability to identify client needs, problem solves and implement timely and appropriate social work interventions.
- Knowledge of community resources and community health care within Huron and Perth Counties.
- Exceptional interpersonal and communication skills to work with a variety of people, taking into consideration varying needs.
- Familiarity with client support software and databases is also considered an asset.
- Must travel between Huron and Perth Counties.
- Valid Driver’s License, Vehicle, and Proof of Insurance required.
