We have an exciting opportunity for a Registered Nurse Clinical Program Coordinator who is interested in joining our fast-paced and dynamic Retinoblastoma team. Retinoblastoma is a rare, but highly curable, childhood cancer that arises from the retina and typically affects infants and young children. The Ophthalmology Clinic's Retinoblastoma Program works collaboratively with internal and external partners to support timely access to care for patients from across Canada. The RN Clinical Program Coordinator will play a central role on the multi-disciplinary Retinoblastoma team, facilitating efficient, high-quality care by coordinating referral intake, diagnostic work-up, treatment planning, procedures and follow-up.

Here's What You'll Get To Do
Key Responsibilities

Coordinate and manage patient care for infants and children undergoing treatment for retinoblastoma, from diagnosis through treatment and follow-up.

Serve as primary point of contact for patients and caregivers, offering education, support and guidance throughout the treatment process.

Develop an effective communication strategy to establish and maintain continuity of care between coordinators, patients/families, surgeons and other team members along the trajectory of care.
Patient Care Coordination
-
Manage referral intake, working collaboratively with the multi-disciplinary team to process and triage new referrals efficiently.

Collaborate with the multi-disciplinary tram to develop individualized, evidence-based care plans.

Collaborate with the multi-disciplinary team to ensure timely completion of pre-operative testing and consents, to prevent surgical delays.

Maintain an up-to-date, centralized registry of all active retinoblastoma patients, tracking each active patient's status, including outstanding testing/assessment results and planned next steps.
Enhance Patient and Family Education

Coordinate with the multi-disciplinary team to support clear and timely communication with families/caregivers about planned appointments, procedures and next steps.

Provide family-centered teaching (ex., pre- and post-operative instructions), setting realistic expectations for the treatment experience and recovery, empowering caregivers to actively participate in care and promoting adherence to post-surgical protocols (as applicable).

Identify barriers to care and escalate to Social Work, as appropriate.

Support optimal patient experiences by collaborating and coordinating with Child Life and other supports, as needed.
Improve Post-Operative Care Coordination
-
In collaboration with the multi-disciplinary team, facilitate discharge planning and follow-up to minimize length of stay and prevent complications.

Support patient compliance with post-operative care and serve as a liaison between patients, families/caregivers and providers.
Here's What You'll Need

Graduate of an approved Baccalaureate Nursing Program
-
Registration with the College of Nurses of Ontario as a Registered Nurse
-
Minimum 3 years experience in pediatric nursing, ambulatory care preferred; established experience in Ophthalmology is preferred
-
Basic proficiency with computers is essential (i.e. word processing, excel and computerized documentation system (Epic preferred)).

Strong communication, interpersonal and organization skills.

Demonstrated problem solving abilities, critical thinking skills and ability to work within a team.

Ability be self-directed, flexible and to take initiative.
-
Attention to detail and efficient time management are essential to the position.

Basic understanding of change management processes, exposure to project planning, and an interest in quality improvement are considered assets.
