Position: COMMUNITY MENTAL HEALTH WORKER

Salary: $27.54 to $34.43 per hour
Hours: 37.5 hours per week Must be available to work weekdays, weekends and stat days on a rotating basis
Status: TEMPORARY FULL-TIME 1 year
JOB SUMMARY:
The Assertive Community Treatment (ACT) program provides treatment, rehabilitation and support in the community to adults with severe and persistent mental illness and a primary diagnosis of a psychotic disorder. Its multidisciplinary team typically includes a Psychiatrist, Psychiatric Nurses, Social Workers, Occupational Therapist, Community Mental Health Workers, Peer Support Worker, Psychiatric Resident, as well as students from different disciplines.
Goals: To reduce/minimize the impact of mental illness and the number of visits and length of hospital stay; to facilitate the integration of clients into communities of their choice; and to help clients improve the quality of their lives, in the areas of health, housing, finance, and vocation.
Services: An individualized plan of service is formulated with the client, family or significant others, based on identified needs/goals. Service plans are regularly reviewed and adjusted to arising needs. The services include, but are not limited to: continuous assessment of client mental health status; adjustment and delivery of medication; monitoring clients physical health; linking to appropriate resources; attaining and maintaining housing; exploring vocational abilities and possibilities; accessing and using available financial resources; and developing and improving personal living skills and activities of daily living.
QUALIFICATIONS:
B.A. or recognized Human Services, Social Services diploma program or equivalent
Completion of a Basic Cardiac Life Support (B.C.L.S.) program
Two years current experience working with psychiatric/addictions patients
Current experience working in community-based psychiatry
Proven knowledge of psychosocial rehabilitation
Experience in substance abuse treatment an asset
Strong time management, problem solving and organizational skills
Effective written and verbal communication skills
Experience working in a multi-disciplinary team environment an asset
Proven crisis intervention skills
Demonstrated involvement with people who have severe and persistent mental illness
Ability to work effectively and efficiently under pressure
Knowledge of mental health policies, legislation, (ie. Occupational Health & Safety Act, Mental Health Act) and other relevant legislation
Knowledge of community resources pertinent to consumers of the mental health system
Computer literacy
Vaccines (COVID-19 and others) are a requirement of the job unless you have an exemption on a medical ground pursuant to the Ontario Human Rights Code.
