Position Alternate weekends day hours starting Saturday from 8am-8pm and Sunday 8am-5pm
This is located in Spruce Grove. Vehicle required to transport individuals as needed. The individuals who will be support are youth and young adult. The successful applicant will have lots energy and create a fun atmosphere.
Position Title: Community Support Worker 2
Responsible To : Team Leader and/or designate
Community Support Worker 2 may be required to undertake all the duties as per the Community Support Worker level 1 position description in addition to the duties as described below.
Summary of the broad purpose of the position
To provide a positive and supportive environment for people with a disability that facilitated personal growth and development and which reflects an individual’s cultural and spiritual diversity
To provide opportunities that enables an individual to reach their own potential as a valued member of their community
The Support Worker 2 will work to the vision, mission and philosophies’ of CommunityAim and act as a role model to other team members which reflect the principles, policies and procedures of CommunityAim.
Accountability and reporting/working relationships
The Support Worker 2 is responsible to the Coordinator through the Team Leader and/or designate.
Special Conditions
- Must be willing to work schedule shifts designated.
- Successful applicants for all positions within CommunityAim as per CommunityAim policy must provide a satisfactory Criminal Record Check, Child Welfare check and pass a pre-employment screening.
- Must hold a Current Alberta Class 5 Driver’s license if transportation of individual is required
- Access to own roadworthy vehicle with appropriate insurance that meets CommunityAim policy if transportation within hired program is required.
- Must hold a current First Aid/CPR certificate at a satisfactory level that meets CommunityAim policy.
- Must be physically, emotionally and medically able to lift, transfer, undertake manual handling and be able to perform any aspect of care to the individual supported as required.
- Willing and able to utilize community activities - Swimming, gym workouts, biking to enhance individuals health.
Principal Outcomes, Activities/daily responsibilities
In consultation with the Team Leader and/or designate ensure that work practices are consistent with:
● The philosophy, policies, and procedures of CommunityAim, which holds individuals with disabilities as valued members of their community.
● Follow standards outlined through “Creating Excellence Together”
● Work under the legal and ethical requirements of service delivery including all aspects of care.
Ensure individual health needs are met by:
● Liaising with health care professionals (i.e RN’s, GP’s, Specialists, as appropriate in conjunction with the Team leader and Manager.
● Maintaining contact with and utilizing Community based health care facilities
● Monitoring and implementing Individualized Support Plans as appropriate.
● Ensuring accurate recording of the administration of prescribed medications and other health related documentation.
● Maintaining the domestic environment in a manner that promotes optimum health conditions.
● Attending and participating in regular team meetings and training events.
Implement strategies designed to meet individuals goals, which may include:
● Teaching and assisting individuals with self – help skills i.e. personal care
● Assisting clients in a range of household chores, budgeting, and banking, shopping, attending appointments and recreational and social activities.
● Providing support in establishing community and social networks for the individual.
● Maintaining a positive commitment to individuals by assisting them to achieve their personal goals.
Support and assist the individual(s) to maintain their home in a manner that reflects their values as a member of their community by performing household duties which may include:
● Preparing meals
● General house cleaning , interior and exterior.
● Gardening
● Laundry, mending and ironing.
● Ensuring the maintenance of adequate household supplies, replacing as required.
● Assisting individuals to maintain health, safety, and welfare of pets.
● Assisting in reporting maintenance/repair requirements to CommunityAim.
Financial Records
Where financial/budget protocols are in place-
● Ensure accurate recording of household income and expenditure which complies with each individual budget protocol.
Contribute to the organizational development of CommunityAim by:
● Delivering quality services that are consistent with the principles, standards and service philosophy of CommunityAim.
● Participating in organizational and personal development including participation in training
● Providing ongoing evaluation and feedback on the quality service delivery which contributes to the continuous improvement process.
● Contributing to the development of organizational culture that is supportive of change to better meet the goals of individuals and the organization.
● Carrying out duties and operations ethically , fairly, and within statutory, legal and contractual requirements.
● Ensuring that all policies, procedures and delegations are fully understood, implements and complies with in accordance to CommunityAim.

● Take necessary precautions to avoid compromising the health and /or safety of others.
● Exercise a Duty of Care in providing services to individuals

Essential Minimum Requirements
Minimum 3 years experience in a field acceptable to CommunityAim
or
Minimum 1 years experience in a field acceptable to CommunityAim plus a college/university diploma
