The Registered Practical Nurse (RPN) is a member of the front line support team and is responsible for supporting individuals in reaching their full potential as a member of our community and in all health related matters. The RPN advocates for individuals so they may live in a state of dignity, share in all elements of their community and have the opportunity to participate effectively. All supports shall be provided in a manner that promotes the care, welfare, safety, and security of the individual.
Under the clinical direction of the Clinical Care Coordinator [RN], the RPN is responsible for providing health supports and interventions across all programs at out Leitrim location in accordance with healthcare practitioner orders, the practice standards of the College of Nurses of Ontario and accepted practice guidelines. The RPN is also responsible for communicating the health needs of individuals to our front line team, assisting clients in all activities of daily living and to support those staff in making appropriate decisions when providing care.
RELATED TO INDIVIDUALS RECEIVING SERVICES AND SUPPORT:
Promotes equal respect for clients within the respite program and community through:
- enhancing public awareness through respectful and dignified interactions;

- educating individuals regarding their responsibilities in maintaining respect; and
maintaining individuals’ and business confidentiality
Provides nursing care in accordance with administrative policies on health maintenance, health care practitioner orders, provincial legislation, and established RHPA, CNO and QAM standards.
Assesses a client’s condition by recognizing and interpreting signs and symptoms, and where appropriate, initiates remedial health measures.
Reports any health status changes to the Clinical Coordinator and supports front line staff in the assessment of clients who become actively ill while in our care.
Advocate for change if a prescribed practice has the potential to put the health and well-being of the individual at risk.
Maintain accurate and up to date client related records including but not limited to:
- develops and maintains up to date individual client care plans through interdisciplinary care conferences and care planning;
- maintains accurate and complete clinical records on all clients according to established documentation policies and procedures and ensures the confidentiality of these records;
- ensures the recording and safe keeping of all drugs including controlled drugs and narcotics;
- ensures orders are correctly processed, recorded and followed through according to established policies.
Assist clients in understanding and exercising their rights in a safe and responsible manner.
Assist clients in all activities of daily living and in fulfilling the requirements of their care plan.
Assist with managing financial resources of the organization through monitoring purchases such as medical and personal supplies, etc.
Maintains professional conduct and communicates effectively.
Discusses necessary information with co-workers at beginning and conclusion of scheduled shifts as well as maintains communications (through communication log and online communication book) and cooperates with team members during shift and at team meetings.
Keep up to date with all agency information through reading emails, reading memos, reading communication log posts prior to each shift.
Participates on committees requiring a nursing perspective while attending all regularly scheduled nursing meetings: IPACC, Health and Safety, etc.
Assists the Supervisors, and Clinical Coordinator with all activities related to Infection Control.
Attends regular in-services to remain up to date on skills and actively participate in nursing learning plan set out by the College of Nurses.
Works all shifts as assigned and ensure availability is updated on a regular basis.
Completion of daily tasks that could include, daily checklists; all chores or tasks for their shift, flow sheets for clients in their care, updates to care plan when needed, clothing lists, flow sheets and other reports as required.
Reports deficiencies in repair of site, equipment and furnishings and assist in rectifying deficiencies when deemed appropriate.
Maintains the organization of and up-keep of medical supplies and nursing related documentation.
Supports the Clinical Coordinator in teaching and monitoring unregulated care providers who are performing controlled acts and advanced health care skills.
Identify and report any practices or directions that go against medical best practices or could have an adverse effect on the health and well-being of the individual. This includes but is not limited to prescribed care plans, medication schedules, nutritional plans, treatment plans, intervention plans.
Other related duties as required.
A Registered Practical Nurse in good standing with the College of Nurses of Ontario.
Proof of Professional Liability Insurance with RPNAO or equivalent.
Demonstrated leadership ability with comprehensive knowledge of nursing and health care practices and techniques as they relate to developmental services.
Maturity and good physical and mental health, empathy, patience and tact are important qualities to possess.
Seeks out opportunities to maintain competency and continued learning.
Proficiency in a second language consistent with the client population is an asset.
Pediatric experience or relevant experience is an asset.
