Adult Protective Services Worker
Who We Are:
Tri-County Community Support Services (TCCSS) is a FOCUS Accredited developmental services agency that provides specialized supports and services for individuals, families and service providers that contribute to the social inclusion and quality of life of persons with developmental disabilities and/or Autism Spectrum Disorders (ASD). The APS program has offices in Peterborough, Cobourg, Campbellford and Kawartha Lakes.
Where We Live and Work:
Located in the beautiful Kawarthas, Peterborough, Ontario is an hour and a half drive East from Toronto, Ontario. There is Trent University and Fleming College in our area as well as many new and growing businesses. We are similar to the Muskoka Lakes region with all of our natural beauty, green outdoor space and tranquil waterways.
Assisting individuals to identify goals and priorities and supporting them to develop case management plans using a strength based approach with the goal of supporting them to attain the maximum independence possible
Assessing the individuals capacity in activities of daily living, accessing supports to address areas of need and supporting them to develop needed skills
Safeguard the rights of individuals either by advocating on their behalf with their agreement, or encouraging and providing opportunities for individuals to learn how to become self-advocates
Participate in case consultations, interagency committees and other community initiatives
Travel throughout our catchment area with reimbursement for eligible mileage
Successful completion of Undergraduate Degree in Social Science or related field
2-5 years Case Management experience working with adults in developmental services
Knowledge of relevant legislation and community resources along with a developed network
Strong conflict resolution and organizational skills
Ability to work flexible hours
Computer proficiency and excellent communication skills
Compassionate and responsible supporter of people, their families, and community partners
Commitment to maximize resources and improve people's quality of life
A valid driver's license and insurance, access to a reliable vehicle and ability to drive in various weather conditions
