Position Overview
We are seeking a compassionate, reliable, and professional Mental Health Support Worker / Community Care Aide to join our community outreach team. This role involves travelling to clients' homes to provide medication witnessing and supportive wellness check-ins for individuals living in the community with mental health challenges.
The successful candidate will be responsible for building positive relationships with clients while ensuring medication adherence and reporting any concerns to the healthcare team.
Key Responsibilities
- Travel to client residences using a company-provided vehicle.
- Witness and document prescribed medication administration.
- Engage clients in a respectful, supportive, and professional manner.
- Monitor client well-being and report concerns to the supervising healthcare team.
- Maintain accurate records and documentation.
- Follow all privacy, safety, and organizational policies.
- Promote client dignity, independence, and recovery-oriented care.
QualificationsRequired
- Valid Class 5 Driver's Licence with a clean driving record.
- Excellent communication and interpersonal skills.
- Professional, dependable, and compassionate approach to client care.
- Ability to work independently and manage time effectively.
- Comfortable working in community settings and travelling between client homes.
Preferred
- Experience working with individuals experiencing mental health, substance use, or social challenges.
- Health Care Aide, Community Support Worker, Mental Health Worker, or related education/training.
- First Aid/CPR certification.
