About The Role
The Nurse Practitioner will provide comprehensive primary and preventative care across diverse clinical, facility, and home-care settings. Working collaboratively with physicians, case managers, and other healthcare professionals, you will assess patients, manage complex medical needs, develop individualized care plans, administer treatments, and support positive patient outcomes. Other duties may include:
- Conducting Comprehensive Care Through Patient Assessments, Performing Physical Examinations, & Developing Individualized Care Plans
- Ordering, Performing, & Interpreting Diagnostic Tests
- Assisting With & Performing Procedures Such as Phlebotomy, Laboratory Testing, & Pain Management Techniques
- Prescribing & Administering Medication Accurately, Strictly Following Medical Orders, Provincial Regulations, & College Standards
- Monitoring Patients' Vital Signs & Responding Promptly to Changes in Clinical Condition
- Managing Specialized Care Regarding Chronic Conditions, Pediatric Health Issues, & Senior Care Needs
- Supporting Patients with Dementia or Other Cognitive Impairments by Implementing Specialized Care Strategies
- Coordinating Collaboration & Case Management Seamlessly with Physicians, Case Managers, & Other Healthcare Professionals to Optimize Patient Outcomes
- Educating Patients & Families Regarding Health Conditions, Medication Use, Post-Discharge Care, & Healthy Lifestyle Choices
- Documenting Patient Information Accurately Using Electronic Health Record (EHR) Systems Such as Epic or Cerner
- Maintaining a Safe, Clean Environment While Strictly Adhering to Infection Control Standards
- Other Tasks as Assigned
Qualifications
- Must Not Have Been Employed as a Nurse in Ontario Within the Six Months Prior to the Date of Hire.
- Master’s Degree in Nursing (Nurse Practitioner Stream) From an Accredited Program
- Current, Unrestricted Registration as a Nurse Practitioner (NP) in The Extended Class with The College of Nurses of Ontario (CNO).
- 3–5 Years of Clinical Experience.
- Strong Knowledge of Medical Terminology & Medication Administration Protocols. Experience with EHR Systems Like Epic or Cerner is Highly Preferred.
- Skilled in Case Management, Pain Management, Phlebotomy, Laboratory Testing, & Dementia/Behavioral Health Care.
- Strong Organizational Skills, Sharp Attention to Detail, & The Ability to Work Effectively Within Multidisciplinary Teams in Fast-Paced Settings
