Position Summary:
- The Canadian Mental Health Association, Simcoe County Branch seeks a Registered Practical Nurse (RPN) provides client-centred, recovery-oriented care within a multidisciplinary mental health team, supporting individuals with complex mental health and substance use needs. The role involves conducting comprehensive assessments, administering and monitoring medications, and developing and implementing individualized care plans to promote stabilization, rehabilitation, and community integration. The RPN delivers direct nursing care, counselling, crisis intervention, and psychoeducation, while supporting daily living skills, health promotion, and relapse prevention. Working collaboratively with internal teams and external partners, the RPN coordinates care, advocates for client needs, and ensures continuity of services, while maintaining accurate documentation and adhering to professional, legal, program model of care, and organizational standards.
What You'll Do:
- Health teaching, consult and recommendations
- In accordance with program needs, Perform all aspects of nursing care within the professional scope of a Registered Practical Nurse (RPN) in accordance with the standards of practice established by the College of Nurses of Ontario, organizational policies, and the program’s model of care.
- Function as an active member of an interdisciplinary team, delivering recovery-oriented, client-centred services in collaboration with internal and external health and community service providers.
- Conduct comprehensive physical and mental health assessments, including monitoring symptoms, behaviours, risk factors, co-occurring conditions, and response to treatment.
- Develop, implement, monitor, and evaluate individualized recovery and care plans (e.g., OCAN), coordinating interventions that support mental health stabilization, daily functioning, housing retention, health needs, and recovery goals.
- Provide direct nursing care and client support, including therapeutic interventions, counselling, psychoeducation, crisis intervention, health promotion, medication education, and recovery-focused services.
- In accordance with program needs, administer, monitor, and document medications, ensuring medication compliance, reconciliation, therapeutic monitoring, and coordination with prescribers, pharmacies, laboratories, and other health-care providers.
- Review, interpret, transcribe, and communicate physician orders, diagnostic results, laboratory findings, and changes in client status in a timely and accurate manner.
- Monitor blood work and diagnostic testing, including clozapine monitoring requirements, follow up on abnormalities, and communicate concerns to the clinical team and prescribing providers.
- In accordance with program needs, provide intensive community-based treatment, rehabilitation, outreach, and support services to individuals experiencing mental illness and complex needs.
- In accordance with program needs, support medication management as part of physician- and nurse practitioner-led treatment plans, which may include observing and documenting medication administration and adherence, monitoring therapeutic response and side effects, and reporting clinical concerns as appropriate.
- Deliver rehabilitation and skill-building supports that enhance activities of daily living, social functioning, health management, and community integration.
- Facilitate and lead therapeutic, psychoeducational, wellness, and skill-building groups as required.
- Provide education, consultation, advocacy, and support to clients, families, caregivers, colleagues, students, and community partners regarding nursing, mental health, recovery, and health-management needs.
- Coordinate care and consult with internal and external service providers to promote continuity of care, effective communication, and successful transitions between services.
- Develop and maintain collaborative relationships with hospitals, primary care providers, pharmacies, community agencies, and other service providers to support integrated client care.
- Support clients in obtaining and maintaining housing, health services, and community resources through assessment, advocacy, system navigation, and referral activities.
- Participate in risk assessment, risk management, care planning, clinical documentation, quality improvement activities, and interdisciplinary case reviews.
- Provide crisis response, de-escalation, and stabilization support during working hours and contribute to safety planning as required.
- In accordance with program needs, coordinate and support clinic and health service operations, including scheduling appointments, coordinating service delivery activities, maintaining equipment and supplies, and liaising with physicians, Nurse Practitioners, pharmacies, and community partners.
- In accordance with program needs, support the planning, implementation, evaluation, and continuous improvement of health and clinical services to ensure accessible, client-centred, and integrated care.
- Serve as a clinical liaison with pharmacies, health-care providers, and community partners to support medication management, communication, and continuity of care.
- In accordance with program needs, provide guidance, support, and coordination to peer support workers, students, volunteers, and other team members involved in service delivery.
- Provide clinical and administrative support to physicians, Nurse Practitioners, and other health-care providers, including chart reviews, quality assurance activities, and other delegated responsibilities within scope of practice.
- Maintain accurate, timely, and comprehensive clinical documentation in accordance with professional, organizational, and legislative requirements.
- Transport clients within the community and, when required, outside the geographical area in accordance with organizational policies and procedures.
- Participate in team meetings, clinical supervision, professional development, organizational initiatives, and continuous quality improvement activities.
- Perform other duties as assigned by the Program Manager or organizational leadership.
Required Qualifications:
- 1-3 years experience in psychiatric nursing (community-based preferred).
- A Registered Practical Nurse registered as a member in good standing with the College of Nurses of Ontario.
- Submission of a Vulnerable Sector Check (VSC), including a Criminal Record Check (CRC) in good standing.
- Must have access to a vehicle, a valid Ontario driver’s license, and adequate automobile insurance.
- Fluency in French or another second language would be an asset.
- Legally entitled to work in Canada.
Salary Scale:
- RPN: $61,600 - $70,000 *per annum
Hours of Work:
- Thirty-five (35) hours per week.
- Rotational shift work based on a seven (7) day staffing schedule.
- Program provides service 365 days per year.
- Able to work within a flexible framework to respond to client needs.
