Reporting to the Manager of Patient Flow and Navigation, the Patient Flow Coordinator will act as a resource to patients, families and employees of the BCHS by providing consultation in situations where advanced expertise is required. You will provide advice and guidance to other healthcare providers regarding complex patient care matters, planning, policy development, policy administration and problem solving. The primary goal of the Patient Flow Coordinator is to facilitate and enhance patient flow organizationally and to mobilize teams to achieve throughput. The Coordinator does this with the goal of providing an exceptional experience for patients, families and staff. The Coordinator understands the demand and capacity challenges of a hospital system and works to achieve patients being in the right place, at the right time, receiving the right care.
Current certificate of competence as a regulated healthcare professional
Minimum of three years of demonstrated experience in a leadership role
Undergraduate degree in a related field is a minimum requirement
Ability to work within a fast paced environment as a reliable, collaborative interdisciplinary team member
Demonstrated ability to liaise/communicate with external stakeholders and internal corporate staff members in a professional and courteous manner.
Excellent written skills to compose abstracts, write proposals and develop poster presentations
Project management experience an asset
Self directed and able to deal with multiple stakeholders while managing multiple priorities.
Excellent leadership skills and the ability to coach, and motivate the healthcare team
Ability to work effectively and collaboratively with the leadership group on unit specific, corporate and external projects.
Facilitates patient flow and utilization within appropriate resources. Makes patient flow decisions and reallocation of resource decisions based on safety and awareness of overall organization needs.
Demonstrates and role models values consistent with mission, vision and values of the organization.
Supports staff in utilizing critical thinking skills in resolving system-wide operational, clinical and administrative issues.
Accepts responsibility for own performance and continuing education/training to maintain clinical competency.
Participates on various program, hospital and community-based committees as required.
Supports staff in utilizing critical thinking skills in order to resolve clinical and administrative issues.
Addresses day-to-day operational issues to ensure optimal patient flow is achieved
In addition to being one of the key points of contact for the organization, the Patient Flow Coordinator is also responsible for developing a flexible, responsive system to meet changing and competing demands
