Job Overview
We are seeking a dedicated and patient-focused Pharmacist to join our healthcare team. The ideal candidate will be responsible for ensuring the safe, effective, and accurate distribution of pharmaceuticals, requiring a strong understanding of medical terminology, medication administration, and patient communication. This role involves both dispensary responsibilities and collaboration with healthcare providers to optimize patient outcomes.
The Pharmacist will oversee prescription verification, provide clinical guidance on complex therapies such as Opioid Agonist Therapy (OAT) and Hepatitis C treatment, and support medication management for supportive housing and complex care environments. A strong understanding of pharmaceutical care, medication safety, and regulatory compliance is essential.
Responsabilities
- Review and verify prescriptions for accuracy, appropriateness, and safety.
- Provide clinical oversight for medication dispensing, including OAT, Hepatitis C treatments, and compliance packaging (e.g., strip packaging).
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care plans.
- Ensure compliance with all provincial and federal pharmacy regulations and standards of practice.
- Monitor and manage medication therapy, identifying and resolving drug-related problems.
- Maintain accurate records of prescriptions, controlled substances, and inventory.
- Provide drug information and education to healthcare team members as needed.
- Participate in quality assurance initiatives and continuous improvement activities.
- Oversee inventory management, including ordering, storage, and handling of medications.
Qualifications
- Licensed Pharmacist in good standing with the provincial regulatory body (e.g., College of Pharmacists).
- Previous experience in a community, hospital, or long-term care pharmacy setting is preferred.
- Knowledge of medical terminology and medication administration processes.
- Familiarity with critical care practices and acute care environments is a plus.
- Excellent attention to detail and strong clinical judgment.
- Ability to work effectively in a fast-paced environment while maintaining high standards of patient service.
- Effective communication and collaboration skills.
