South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. As a member of a multi-disciplinary SOS team, the Trauma Social Worker will provide 1:1 therapy/counselling, crisis support and case management for individuals impacted by both trauma and substance use. Community development and group work facilitation will be additional components of this role as well as complementary support to teams working with this community.
This position will be required to work some flexible hours, including some evenings and weekends. This position will also be involved in shelter outreach work and working with clients who are COVID-19 positive. Working onsite during the COVID-19 pandemic is required.
Responsibilities:
- Conducts client intakes, assessments, care planning and implementation
- Responds to crisis situations and facilitates internal and external referrals as appropriate
- Develops client-centred, individualized care plans, including appropriate interventions and safety planning as required
- Provides 1:1 counselling services around a range of concerns, including grief, trauma, substance use, suicide, health-related matters, relationships, depression, anxiety, and gender-based, age-specific, racial/cultural violence and discrimination
- Participates in care at off-site locations/partner agencies as well as visits to clients who live within the catchment area as necessary
- Works as a high functioning member of an interdisciplinary team
- Participates in client care conferences with other providers to ensure coordination of services to the client
- Identifies client needs that are best addressed through group-based interventions
- Collaborates with colleagues and partners to plan and facilitate group sessions
- Applies an equity lens to interdisciplinary care
- Advocates for client rights and benefits – as individuals, groups or communities
- Participates in all aspects of program design, implementation and evaluation
- Ensures that standards of client care are maintained according to organizational and College requirements
- Collaborates with own team and other teams across the organization to identify and provide health education to target groups
- Other related duties as assigned
Qualifications:
- From a recognized university, a Masters of Social Work (MSW) or Bachelors of Social Work (BSW) with advanced training/experience in a trauma related field
- 3-5 years’ experience in a community-based human service and/or health organization
- Member in good standing with the Ontario College of Social Workers and Social Service Workers (or other regulating body)
- Demonstrated understanding of the intersection of trauma, mental health challenges and substance use
- Advanced experience working in harm reduction and with those who are substance using
- Advance experience/training using a range of therapeutic approaches incorporating a harm reduction, client-centred framework
- Excellent knowledge of mental health issues, particularly within equity deserving communities
- Ability to deal effectively in the prevention and management of crises
- Thorough knowledge and understanding of community resources
- Skilled in group facilitation
- Demonstrated ability to work in a multidisciplinary team environment
- Demonstrated respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others
- Demonstrated experience working across all genders with racialized populations, indigenous peoples, women who engage in sex work, individuals who are/have been incarcerated, people with mental health challenges, and people who have experienced trauma
- Proficiency in the use of computer technology and various software applications
- Strong oral and written communication skills
- Excellent interpersonal skills
- Fluency in a language relevant to the South Riverdale community is a benefit, in addition to English
- Police clearance documentation will be required for employment
- Proof of triple COVID-19 vaccination status will be required for employment
