Under the direction of the Manager of Kidney Health , the social worker assists the healthcare team in the development of Kidney Health. The incumbent assesses and provides counseling to clients and their families dealing with the complexities of kidney disease. The social worker will function as a member of the renal program with an emphasis on the team approach to care. Travel is required for this position. The Social Worker will function as a member of the Kidney Health interdisciplinary team, with an emphasis on patient and family-centered care.

Job Qualifications
Required Qualifications

Bachelor Degree in Social Work
Registered or eligible for registration with the Saskatchewan Association of Social Workers (SASW)

Experience
Previous experience with medical social work in the field of chronic disease management is required.

Field Hours
Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement. HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.

Additional Information
Demonstrated ability to develop a quality monitoring program to assess, monitor and improve quality of care. Knowledge and ability in a variety of counseling modalities, in particular, individual, family and group skills. Experience with First Nations and Metis Health practices and understanding of health systems preferred.
