Full-Time Clinical Lead
Hampton Terrace is looking for Registered Practical Nurse to join our care team as Clinical Lead. We are a Best Practice Spotlight Organization designate and a fully Accredited Home with exemplary standards rom Accreditation Canada. Works under the direction of Clinical Manager and oversees nursing care practices to residents in accordance with required medical care & established nursing care plans and practices, including assessment, data collection and tracking of mandatory programs and other incidents related to resident care and safety. Work collaboratively with other members of the team, provide support and coverage in each other’s absence/s. Supervise all activities on the unit/s and provide direction to the RPNs and direct care staff on the unit.
Responsibilities:
- Receive shift report from the Charge Nurse upon arrival and take over the primary supervision of assigned unit/area.
- Follow-up on any concerns within the past 24 hours to include but not limited to pain, falls, infections, wound care, behaviours, resident transfer to hospital, returns from hospital/extended leaves, outstanding lab reports, significant changes in residents' condition, care plan and changes in physician's orders
- Ensures the care plans for assigned residents are developed, updated as needed, and reviewed on a quarterly basis as per established schedule. (on PCC using RAI MDS, including coding, RAPS in accordance with MOLTC legislated requirements).
- Supervise and provide direction to RPNs and PSWs, ensuring that resident's care is provided in accordance with the care plans and Home's polices and procedures are adhered to by assigned staff.
- Ensure effective implementation, compliance, monitoring and evaluation of all mandatory programs in collaboration with Nurse Practitioner/Clinical Manager and interdisciplinary care team to include:
- Work rotation weekends ( 1 in every 5-6)
* skin and wound care / Incontinence care / falls prevention / responsive behaviour/ pain management / restraints/Education and Training - Current registration with the College of Nurses of Ontario. Post graduate studies in nursing, nursing administration and geriatrics an asset. Experience in LTC is an asset Must be in good standing with the College of Nurses
Knowledge - A sound knowledge and background in geriatric, rehabilitation or long term care nursing and personnel administration. Knowledge of the legislated requirements for long term care facilities in Ontario, the appropriate sections of The Health Disciplines Act, Occupational Health & safety Act, Pharmacy Act, Food & Drugs Act (Canada), Narcotic Control Act (Canada) and other pertinent Federal, Provincial and Municipal Statutes, which influence the operation of a long term care facility and more specifically the nursing department.
Attributes - Tactful manner, ability to communicate on an appropriate level with various levels of staff and families. Must be in good physical and mental health. Must be knowledgeable in nursing process, developments and trends in the geriatric care field and have the skill to assist in the planning, coordination and supervision of a comprehensive resident care program.
