In this position, the Clinical Pharmacy Coordinator will support departmental activities related to clinical pharmacy services, to ensure that these services meet the needs of patients and contribute to optimal patient outcomes.
- Participate in patient care rounds and provide pharmaceutical care, including medication reconciliation upon admission and discharge,
- Collaborates with physicians, nurses and other clinicians to enhance patient care.
- Start, teach and monitor patients in the self-medication program
- Provision of Saturday coverage and on-call responsibility in rotation
- Participate as required in hospital committees
- Provide education to patients to promote optimal drug use.
- Act as a preceptor for Pharmacy students
- In collaboration with the Manager of IPAC and Pharmacy, will perform the following:
- Provide new clinical pharmacists with orientation, training and guidance
- Coordinates clinical pharmacy services including the development and implementation of new services, and assures quality of clinical pharmacy programs
- Assist with the development of clinical practice initiatives and formulary management
- Aupport the sustainable delivery of excellent patient-centred care, and safe, effective pharmacy operations
- Act as a leader for pharmacy practice and resource for members of the Pharmacy Team.
- Be aware of conditions and measures that may affect the safety of patients, residents, students, employees, volunteers, physicians and visitors including:
- Compliance with the safety standards of the hospital and applicable legislation
- Identification and prevention of safety meetings/training/updates in accordance with the Centre's standards
- Ability to work in a manner that exemplifies the Centre's core values and helps patients get their life back
- Evidence of good performance history and attendance record
- Graduate of an accredited Faculty/College of Pharmacy (Baccalaureate Degree in Pharmacy or Pharm D.)
- Registered as a Part A Pharmacist with the Ontario college of Pharmacists
- Designation certification an asset, such as geriatric pharmacist, anticoagulation pharmacist, antibiotic stewardship, diabetic educator etc. preferred
- 3-5 years relevant experience in a hospital setting with demonstrated clinical pharmacy leadership
- Computer proficiency
- Teaching experience through clinical mentorship
- Demonstrated experience to coach and mentor clinical pharmacy staff
