Job Summary
St. Jude Community Homes (SJCH) provides supportive housing to people challenged by mental illness. The organization operates on strong principles of community development and recovery to facilitate a supportive community where individuals can live, learn and grow.
Resource Worker, Dual Diagnosis provides support to clients living with a dual diagnosis (co-occurring developmental disability and mental illness), as well as clients living with symptoms of a mental illness. The Resource Worker also works with clients who have dual diagnosis and justice system involvement. She/he is responsible for engaging with each client to provide social and community supports to help them maintain and thrive in their home. This will include developing goal plans, providing service coordination, coaching, skills teaching, safety planning and linking/referral supports aimed at assisting each client to achieve his/her goals.
Resource Worker must collaborate with clients, client families/natural support networks and agencies/services in the mental health, justice, and developmental services sectors to effectively fulfill their role.
Responsibilities
Provides Support Services by:
- maintaining responsibility for a designated resident group
- developing and maintaining a positive, friendly and respectful relationship with all residents, staff and Board members
- engage residents as full collaborators in service planning, delivery and evaluation
- providing a safe environment and community for the residents to live and participate in
- encouraging residents to focus on their abilities and challenge their weaknesses
- facilitating involvement in their home community with other residents
- working in partnership with residents in program planning
- providing understanding and compassion when residents are experiencing challenges
- assisting residents in setting personal goals and providing necessary skills training
- recognizing when residents are experiencing health related concerns and able to address these concerns appropriately
- assisting with resolving conflicts through mediation and dialogue
- providing crisis intervention when necessary and reporting critical incidents to the program manager or designate in a timely manner.
Provides Residents with Information of Community Programs by:
- assisting residents to discover and utilize community resources
- working with residents on planning and facilitating outings/social activities
- when appropriate attending the outing(s)
- fostering community partnerships and ongoing liaison with various agencies
Qualifications
- Degree or Diploma in Developmental Service Worker or Social/Community Services (minimum 2- year program) from a recognized Community College/University in Canada.
- A minimum of two years’ previous experience supporting people with a developmental disability, mental illness, or dual diagnosis.
- Sound knowledge and understanding of the client population: Demonstrated ability to work independently and effectively with individuals with co-occurring developmental disability and mental illnesses. Has the ability to use problem-solving techniques to identify and assess client needs and develop appropriate intervention strategies.
- Knowledge of principles of case management theory and practice: Demonstrated ability to provide holistic and comprehensive case management services to all clients including initial and ongoing assessments, goal setting, short and long-term care plan development, crisis intervention, progress monitoring, individual financial management.
- Communication and interpersonal skills: Demonstrated ability to engage in a meaningful relationship with a broad range of clients including those that are resistant. Skillful in assertive outreach and follow-up using a flexible approach to support/assist clients in remaining engaged.
- Previous service coordination experience is necessary. Have knowledge of community resources and demonstrated ability to negotiate with others on behalf of clients.
- Proficient in MS Office.
- Extensive knowledge of community services and resources relevant to client population will be considered an asset.
- Experience with group facilitation is considered an asset.

Pay: $24.00-$27.00 per hour
