Reporting to the Support Work Program Lead, the Support Worker fosters a safe and supportive environment, with a focus on daily living skills, emotional and psychological needs. The Support Worker plays a crucial role in supporting families on their journey toward resilience and healing.
RESPONSIBILITIES
- Support youth in the development of their daily living skills, based on their needs and goals.
- Collaborate with community resources focused on wellness services and support, which may include cultural teachings, mental health and addictions supports, and other services.
- Conducting assessments to identify the specific needs and goals of individuals and families and developing personalized support plans accordingly.
- Implementing strategies and interventions to promote positive family dynamics and enhance communication and conflict resolution skills.
- Monitoring progress and outcomes of support plans, adjusting strategies as necessary to ensure continued effectiveness.
- Providing in-home support services, including assistance with daily living activities, household management, and transportation.
- Advocating for the rights and well-being of individuals and families, ensuring they have access to resources and services that meet their needs.
- Fostering a safe and supportive environment that empowers families to explore and respond to their emotional and psychological needs.
- Complete and submit required service documentation, including daily logs and incident reports.
- Utilize a relationship-based approach to deescalating and responding to emergencies as they arise.
- Maintain timely and effective communication with the holistic circle of care to support the care of youth.
- Other duties as assigned.
EDUCATION/EXPERIENCE
- Minimum completion of high school; or a combination of education and related experience may be considered
REQUIREMENTS
- Valid Class 5F Driver’s license
- Full access to a safe and insured vehicle; with a minimum of $2,000,000 liability
- Satisfactory Criminal Record, Child Abuse Registry and Drivers Abstract
- Active cellular phone
- Proof of double Covid-19 Vaccination
SPECIAL SKILLS
- Excellent oral communication and interpersonal skills
- Ability to write short or detailed contact notes
- Valid First Aid and CPR Certification preferred
