The Cowichan Primary Care Network is seeking a nurse practitioner to work with a team of practitioners and other health providers to collectively deliver care to patients in the Central Duncan Area, initially working out of the Cowichan Attachment Clinic.
This is a centrally located clinic in Duncan that aims to provide attachment for any community members in Cowichan who currently do not have a primary care provider. This position will primarily focus on those community members experiencing challenges with chronic conditions and will provide care both in the clinic and in the community as needed.
Acting as the most responsible primary care provider, there is a requirement for the practitioner to establish a minimum patient panel (panel size expected to be 1,000).
After hours’ care for patients is inherent in the services. The nurse practitioner will participate in an after-hours call group, sharing after-hours coverage with other practitioners.
