POSITION SUMMARY:
We are looking for a pharmacist to join our Pharmacy Services team!
St. Joseph’s Healthcare Hamilton (SJHH) is proud to provide an exciting and rewarding career opportunity. SJHH is partnering with the Government of Nunavut to provide a unique opportunity to provide inpatient care for people living in Iqaluit and neighboring communities. This is a permanent full-time position based out of SJHH in Hamilton, ON, covering a temporary opening for 12 months at Qikiqtani General Hospital in Iqaluit. Upon completion of term in Iqaluit, candidate will return to Hamilton to work as a clinical pharmacist in an inpatient unit. Future coverage in Nunavut may be required as need arises to cover any leaves/absences.

Salary & Benefits:
We offer a very competitive salary and benefits package. Additional benefits while working in Nunavut include: Nunavut Pharmacist licensure cost, shift premium, housing accommodation costs covered, allowance for daily living expenses, and travel expenses to Nunavut.
QUALIFICATIONS:
- Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited university required
- Registration with the Ontario College of Pharmacists, Part A, in good standing
- Injection and Immunization Certification from an OCP-recognized program is an asset
- Previous hospital experience is an asset
- Hospital Pharmacy Residency is an asset
- Certification or advanced training in disease state management is an asset
- Demonstrates strong customer service orientation and skill working collaboratively with others
- Experience working on or leading quality improvement initiatives is an asset
- Possesses excellent oral and written communication skills
- Excellent attendance and punctuality are essential
RESPONSIBILITIES:
- Provides customer centric care, recognizing both patients and other internal stakeholders (eg: other healthcare providers) as customers
- Provides pharmaceutical care to patients while working under full expanded scope of practice
- Reviews safety and appropriateness of medication orders
- Ensures optimal patient care by identifying, preventing and resolving drug therapy problems
- Oversee tasks performed by Pharmacy Technicians to ensure safe and accurate medication dispensing
- Precepts and mentors students and residents
- Attend and present at staff education seminars
- Performs duties in a manner that demonstrates the employee’s commitment to developing a culture of patient and staff safety through accountability, reliability, trust and team work.
- Uses professional communication and excellent interpersonal skills to work effectively with colleagues within and outside the department
- Effectively manages daily workload using demonstrated ability to prioritize patients and tasks
- Identify and follow up on adverse reactions and medication incidents
- Contributes to the corporate efforts and initiatives, as appropriate, to enhance SJHH’s mission, vision and values, by participating in departmental and hospital projects, committees and task groups as assigned
- Acts as a liaison between Pharmacy Services and other healthcare provides (eg: nurses, allied health staff and physicians) to provide education and to promote pharmacy initiatives
- Maintains confidentiality and privacy standards at all times
- Adheres to all applicable NAPRA, regulatory standards and hospital/Pharmacy Services policies and procedures
- Performs all duties as assigned by Manager
