About Bethell Hospice and Hospice Dufferin
Bethell Hospice and Hospice Dufferin are non-profit organizations in the Hills of Headwaters Ontario Health Team (OHT). They provide exceptional and accessible hospice palliative care services to individual and their families facing life-limiting illness and bereavement. Our person-centered approach to care fosters dignity, respect and comfort for our client and their loved ones in Caledon, Brampton and Dufferin County regions of Ontario. Bethell hospice provides series in a 10-bed residential setting. Hospice Dufferin and Bethell Hospice have community programs which include in-home palliative, bereavement, and grief counselling for children and adults. Our services are provided at no cost for our clients.
Overall Summary of Duties:
This position reports jointly to both Bethell Hospice and Dufferin Hospice. This RSW role has been created to provide psychosocial support for those with suspected or confirmed COVID-19, their loved ones and their formal and informal care givers within the Dufferin County-Town of Caledon region. Reporting to the Psychosocial Spiritual Care Lead at Bethell Hospice the Social Worker – COVID Response is responsible for providing direct client care for community clients and their families to ensure their psychological, emotional, physical and spiritual care needs are addressed in a professional and compassionate manner. The incumbent conducts a thorough assessment, and identifies and secures resources key to supporting the client’s care plan. Case management is foundational to this position, utilizing technology for accurate record-keeping and sound judgment in adjusting care plans throughout the client journey.
Responsibilities and Primary Duties:
· Advocates for the needs, decisions and rights of clients and their families;
· In collaboration with clients and their families, assesses care needs, determine eligibility for services, and develop individual care and service plans;
· Reviews care delivery and adjusts according to changing needs through the course of illness and into bereavement;
· Engages in a collaborative process of decision making that involves the client, family and trans-disciplinary care team to establish relevant and feasible care plans;
· Evaluates clinical and program processes and outcomes to ensure that the needs of clients and families are clearly identified and responded to;
· Applies information gathered in assessments to assist clients and their families to make decisions appropriate for themselves;
· Assists clients and their families to obtain appropriate resources available within the Dufferin-Caledon community
· Manages all supporting documentation including case notes and statistics, preparing analysis and reports of same
· Facilitates support groups for clients, as well as team debriefings for community partners
· Contributes to capacity building among community partners, enhances their ability to provide psychosocial support to clients and families affected by COVID 19
Education & Experience:
· Current registration with the Ontario College of Social Workers
· Bachelor’s Degree in Social Work Required, Master’s Degree in Social Work preferred
· Minimum of three to five years of recent work experience as a Social Worker
· - knowledge of the healthcare landscape in the region an asset
- Advanced counselling skills in a variety of approaches including Narrative Therapy
- Comfortable navigating ambiguity to achieve results
- Skilled in establishing relationships with multiple partners in different locations, and advocating on behalf of the client
- Comfortable working in a variety of healthcare settings (ie. hospital, long term care, assessment centres)
- Direct client care during a pandemic requires flexibility and creativity; the successful candidate will be passionate about generating innovative ideas for programs and services for this unique population
- Advanced counseling skills for both clinical and sub-clinical issues across a wide variety of people groups;
- Able to work effectively with a variety of stakeholders;
- Strong written and oral skills;
- Detail oriented, ensuring accuracy and timeliness of information
- Excellent organizational and time management skills;
- Ability to be professional and use sound judgment in all interactions;
- Competent in Word, Outlook, Excel and PowerPoint;
- Self-starter who thrives in an environment where s/he can be equally successful working independently as well as collaboratively in a team environment;
- Strong interpersonal skills;
- Able to exercise tact and discretion while handling confidential information;
- Problem-solving, negotiation and conflict management skills;
- Experienced in working successfully with a diverse client group.
- Expertise in providing palliative/bereavement counselling services
- Bilingual in French and English
There will be a mix of remote work from home, time in an office in the Dufferin-Caledon region, and travel throughout the region to visit clients and community partners. Incumbent needs to be safety conscious, and comfortable with public health prevention measures to ensure safety of self and clients at all times, e.g. wearing PPE when visiting higher risk COVID environments.
Candidates must obtain Vulnerable Sector clearance prior to starting in this position.
Hours of Work:
- Flexibility required in order to meet client needs/scheduling.
- Use of personal vehicle for work related travel; mileage reimbursement provided.
