Position Summary
As a Licensed Practical Nurse (LPN), you will provide high-quality, resident-centered nursing care while promoting dignity, safety, and quality of life. Working collaboratively with Registered Nurses, Directors of Care, Physicians, and interdisciplinary team members, you will deliver professional nursing services and contribute to positive health outcomes for residents.
Key Responsibilities
- Provide direct nursing care in accordance with resident care plans, organizational policies, and professional standards.
- Administer medications, treatments, and injections safely and accurately.
- Monitor, assess, and document changes in resident conditions and promptly communicate concerns to the interdisciplinary team.
- Complete accurate and timely clinical documentation using electronic health records and medication administration systems.
- Support residents with activities of daily living while promoting independence, dignity, and person-centered care.
- Participate in care conferences and contribute to individualized care planning.
- Provide education and support to residents and families regarding health management and treatment plans.
- Adhere to Infection Prevention and Control (IPAC), occupational health and safety, and emergency response procedures.
- Participate in quality improvement initiatives, staff education, and continuous professional development activities.
- Collaborate effectively with healthcare professionals to ensure exceptional resident outcomes and service excellence.
QualificationsRequired:
- Current registration and good standing with the British Columbia College of Nurses and Midwives (BCCNM).
- Graduate of an accredited Licensed Practical Nursing program.
- Current CPR/BLS certification.
- Strong medication administration and clinical assessment skills.
- Excellent communication, documentation, and interpersonal skills.
- Ability to work independently and within an interdisciplinary team environment.
- Strong organizational and time management skills.
- Commitment to resident-centered care and professional excellence.
Preferred:
- Experience in Long-Term Care, Assisted Living, Continuing Care, or Acute Care settings.
- Experience with PointClickCare (PCC), eMAR, and electronic health records.
- Experience working with seniors and residents living with dementia and complex care needs.
- Additional certifications in Infection Prevention and Control, Dementia Care, or Palliative Care are considered assets.
