Role Overview
We are seeking an experienced Client Care Supervisor (RN/RPN) to oversee client intake, assessments, care planning, caregiver support, and ongoing quality of care. The Client Care Supervisor will help ensure clients are appropriately assessed, caregivers are prepared and supported, and services are delivered safely and according to each client's individualized Care Plan.
This position will begin on a part-time basis, with the opportunity for increased hours and responsibilities as our client base grows.
Key Responsibilities
Client Intake & Care Planning
- Answer incoming client intake calls, gather information about care needs, and provide guidance on appropriate next steps.
- Conduct comprehensive in-home and virtual client assessments.
- Support new client onboarding and determine appropriate care needs and services.
- Develop, implement, and update individualized Care Plans.
- Conduct reassessments when client needs or conditions change.
- Maintain accurate documentation of assessments, visits, and Care Plan updates.
- Assist with matching and assigning appropriate caregivers to clients.
Client & Family Support
- Serve as a primary point of contact regarding client care and service concerns.
- Conduct client follow-ups and quality assurance check-ins.
- Monitor changes in client needs and recommend appropriate adjustments to care.
- Maintain professional and supportive relationships with clients and their families.
Caregiver Training & Supervision
- Assist with the orientation and onboarding of new caregivers.
- Provide training, coaching, and guidance to PSWs and caregivers as required.
- Serve as a supervisory resource for caregivers regarding client care concerns or changes in condition.
- Ensure caregivers understand Care Plans and client-specific requirements before beginning assignments.
- Conduct supervisory visits and address caregiver performance or competency concerns when required.
Quality & Operational Support
- Monitor the quality and consistency of care provided to clients.
- Help ensure caregivers follow established Care Plans and agency procedures.
- Review client documentation and identify areas requiring follow-up.
- Support the improvement of client care processes and service quality as the agency grows.
Qualifications
- Current registration as a Registered Nurse (RN) or Registered Practical Nurse (RPN) in Ontario, in good standing.
- 2+ years of relevant nursing, home care, community care, care coordination, or client management experience.
- Experience conducting client assessments and developing individualized Care Plans.
- Experience supervising, training, or supporting PSWs/caregivers.
- Strong assessment, documentation, communication, and organizational skills.
- Ability to work independently and exercise sound professional judgment.
- Ability to manage sensitive or urgent situations professionally and empathetically.
- Valid Vulnerable Sector Check or willingness to obtain one.
- Valid G driver's licence and reliable insured vehicle.
- Ability to travel throughout Scarborough and other areas of the GTA as required.
- Current CPR/First Aid certification preferred.
Compensation & Schedule
- $35–$40.00 per hour, based on experience and qualifications.
- Part-time hours based on client and operational needs.
- Potential for performance-based bonuses based on individual performance
