Duties
- Deliver comprehensive patient care by monitoring vital signs, administering medications, and assisting with activities of daily living
- Support physicians and registered nurses with clinical procedures such as phlebotomy, wound care, and laboratory specimen collection
- Assist with patient assessments, documentation, and updating medical records using electronic health record systems like Epic or Cerner
- Provide specialized care for patients with dementia, seniors in hospice or assisted living settings, pediatric patients, or those requiring acute or chronic care management
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet each patient’s unique needs
- Educate patients and families on health management strategies, medication usage, and post-discharge instructions
- Maintain compliance with all safety protocols, infection control standards, and state licensing requirements
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice nursing in your state
- Proven experience working in nursing homes, hospitals, or home care environments; experience with dementia care or hospice is highly valued
- Strong knowledge of medical terminology, medication administration protocols, and clinical procedures such as phlebotomy and laboratory testing
- Familiarity with electronic health record systems like Epic or Cerner; previous medical office experience is a plus
- Excellent patient care skills with a compassionate approach to diverse populations including pediatrics and senior citizens
- Ability to work effectively in fast-paced settings requiring acute care or case management expertise
- Experience working with children or in assisted living environments is desirable but not required
