Position Summary- Applicants must be willing to travel between Hythe & Grande Prairie on occasion. Applicants must have availability to cover a range of shifts during the day time hours Monday-Saturday.
(Looking for a casual employee to fill in for regular staff. The successful candidate should have experience working with complex behaviors, mental health and addictions as well as experience working with Individuals that are fully dependent . This includes pushing high back wheel chairs and supporting Individuals with personal hygiene. Employees need to be available to work Day shifts - between Monday-Saturday. Hours can be anywhere between 9am-7pm)
Reporting to the Team Leader, the Community Disability Services Worker is responsible for providing support to Individuals in accordance with Blue Heron’s philosophy, the Principles of Support and the Individual’s Personal Support plan. The Community Disability Services Worker provides support that facilitates the Individual’s growth, development and achievement of personal goals.
The Community Disability Services Worker works closely and in consultation with the Team Leader, Program Coordinator and the Individual’s support team.
1. Specific Duties and Responsibilities
a. Individual Support
- Encourage community involvement for each Individual, providing opportunities for structured and informal involvement.
b. Program Development and Implementation
- In consultation with the Team Leader and/or Community Disability Services Practitioner, participate in the development of programs for the Individual.
- Implement all programs in an environmentally and situation appropriate manner, following identified procedures.
- Complete all documentation as required for each individual program procedure.
c. Community Inclusion
- Represent themselves, the program, and the association professionally and positively within the community as a whole.
- Present Individuals in a positive, respectful manner, maintaining confidentiality.
d. Home and Community Support
- Monitor and/or administer medications, as required.
- Follow daily routines in a respectful manner, ensuring Individuals are as involved as much as possible.
- Ensure the individual’s home is maintained in a safe and healthy manner.
- Ensure all equipment and appliances are in good and safe operating condition.
- Support the Individual in developing independent living skills
- Support the Individual in making safe and healthy choices
f. Budgeting and Financial Management
- Provide budgeting information and support to the Individual
- Support the Individual with their personal finances and budgeting
g. Documentation
- Complete, and keep confidential all documented records and files as per program and policy requirements.
- Ensure all required documentation, including daily log notes, incident reports, medication documentation, data collection, program updates, financial records and reporting, monthly and quarterly reporting etc. are completed and maintained as per policy.
h. Teamwork
- Attend all team meetings, sessions and general staff meetings.
i. Liaison
- Establish and maintain effective and professional relationships with the Individual’s personal supports, including the individual’s guardian, family members, friends and natural community connections.
j. Professional Development
- Identify training and development needs with Team Leader.
- Participate in internal and external training events as approved by Team Leader.
- Maintain the responsibility of ensuring all required professional development is obtained and remains current
k. Other
- Any other duties required from time to time as identified by the Community Disabilities Service Practitioner, Team Leader or Program Coordinator as applicable.
2. Qualifications
a. Education
- High School Diploma
- Foundations or Basic Skills Training
or
- Relevant Certificate or Diploma in Human Services
b.Experience
Six month’s experience in the Human Services field preferred.
