Job Summary
Southlake’s Regional Cardiac Program is the 4th largest in Cardiac center in Ontario that provides care for patients requiring Cardiac surgery, Interventional Cardiology, Electrophysiology and Advanced heart failure management. The program provides tertiary and quaternary cardiac services for residents in York Region and Simcoe-Muskoka and is very proud of the clinical advancements provided to the communities served. Success is based on innovative partnerships and amazing people, who represent every aspect of our many services. With a team of more than 3,000 employees, 580 physicians and more than 900 volunteers, Southlake is committed to creating an environment where the best experiences happen.
Engage in direct and in-direct advanced clinical practice, education, research and consultation within the Structural Heart Program to ensure the highest quality of care is provided to patients and families.
Actively participate as part of the Structural Heart Program as well as the professional leadership team in promoting quality care by implementing evidence based practices, evaluating patient outcomes, development and evaluation of policies, clinical programs and services.

- Demonstrated ability to work in a collaborative practice model with inter-professional teams
- Demonstrated ability to use initiative and work autonomously utilizing critical thinking skills and advanced decision making, problem solving skills
- Experience in clinical nursing and inter-professional leadership and education
- Experience with program development, quality improvement initiatives and program evaluation

Qualifications
- Master in Nursing/Nurse Practitioner Stream OR/
- Master in Nursing/Mast of Science in Nursing with successful completion of a post Master’s Acute Care Nurse Practitioner program from a recognized school of graduate studies
- BCLS required
- Minimum 3 years’ experience as advanced practice nurse in Structural Heart preferred
- Minimum 3 years’ relevant clinical specialty-related experience as a Registered Nurse
- Registered Nurse in the extended class with Ontario Certificate of Registration with the College of Nurses of Ontario
- Current Member of the RNAO
- Nurse Practitioner’s Association of Ontario
