Nurse Practitioner (Permanent Full Time)
Position Summary:
Reporting to the Director, Resident Services and working closely with the Medical Director and all attending Physicians and specialists within an inter-disciplinary team, the Nurse Practitioner responds to the specialized needs of the residents. The Nurse Practitioner will support the registered staff and the inter-disciplinary team in all aspects of clinic care, leadership, education, and research. This will include providing education, mentorship and guidance in preventative care and prescribed treatments, conducting annual physicals, ordering clinical and diagnostic tests, and prescribing medication and treatment.
Skills & Qualifications:

Minimum one year clinical and/or working experience in geriatric, rehabilitation, or long-term care nursing.

Leadership knowledge and/or experience are considered an asset.

Certifications obtained or in progress that includes gerontology, mental health, behavioral support and palliative care is an asset.

Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice.

Working Knowledge of the; Long Term Care Act, Program Standards for Long Term Care, Health Disciplines Act, Occupational Health & Safety Act, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada) and Coroners Act, is considered an asset Must be available to be on site.
Duties & Responsibilities:

Provide clinical leadership support for the home’s interdisciplinary team to provide proactive screening and assessment, timely episodic care and specialist referrals follow-up care, ongoing chronic disease management, palliative and end-of-life care.

Provide leadership and mentoring support to staff to enhance their knowledge, assessment skills and ability to care for residents, optimizing resident health care outcomes

Provide care for a select group of residents in consultation with the physicians and the interdisciplinary team
- Participate in the development and delivery of clinical skills education for staff, residents, and families.

Participate in creating an organizational environment that supports safety and quality of resident care and life, collaborative practice, and professional growth.

Maintain current knowledge of care-related practice, issues and activities at Villa Colombo.

Collaborate with the Nursing Leadership Team to identify and implement clinical best practices and develop and roll-out projects and initiatives, including communicating and disseminating evidence-based literature to employees.

Provides input into staff performance and identifies and implements performance plans.

Participate in employee orientation/training to ensure ongoing development of clinical skills and a commitment to the philosophy, values, policies, and clinical practices.

Participate in identifying, analyzing, and interpreting trends in resident care and professional nursing practice to determine priorities for educational programming for team members, residents and families.

Develop linkages with external partners, as appropriate, to ensure continuity of care

Participate in Quality Committee meetings; lead and support CQI projects and initiatives and participate in the implementation of Clinical Quality Improvement Plans

Evaluate outcomes of care and quality programs and make recommendations for change in consultation with the Director, Resident Services, Medical Director, and attending physicians.
- Attend and/or support internal and external committees as required.

Ensures competencies and clinical practice are within standards, practice guidelines, and policies of the Ministry of Long-Term Care, Villa Colombo, and the College of Nurses Standards.

In keeping with professional standards of practice, obtains and/or disseminates information which is relevant to meeting the residents’ identified needs and with appropriate authorization, either in the form of signed consents or direct authorization from the resident or designated substitute decision-maker.
- Utilize expert communication skills by being a link between the resident, consultants, resources, and the referral source, which may be required to coordinate the provision of service to the resident.
- Works in a manner to ensure safety for oneself, clients and their families, colleagues, and the work environment.
