Nurse Practitioner (NP) - Primary Care Provider
Position Summary:
The Elden of Bradford is a retirement home with 148 suites over 6 floors, encompassing Independent, Assisted, and Transitional Care Living.
As a Nurse Practitioner (NP) working as a third-party contractor in a retirement home setting, you will provide high-quality, patient-centered care to a diverse population. You will work collaboratively with staff to ensure comprehensive care delivery. This role involves assessing, diagnosing, and treating a wide range of acute and chronic conditions, as well as promoting health and wellness.
Key Responsibilities:
- Patient Assessment and Diagnosis:
- Conduct thorough physical examinations and obtain comprehensive medical histories.
- Order, interpret, and follow up on diagnostic tests and lab results.
- Treatment and Management:
- Develop and implement individualized treatment plans based on patient needs and best practices.
- Prescribe medications and other therapeutic interventions within the scope of practice.
- Care Coordination:
- Collaborate with specialists and other healthcare professionals to coordinate patient care.
- Participate in patient care conferences and multidisciplinary team meetings as needed.
- Patient and Family Communication:
- Engage with patients and their families to address health concerns and provide emotional support.
- Ensure effective communication of treatment plans and follow-up care instructions.
- Documentation and Compliance:
- Maintain accurate and timely medical records in accordance with legal, regulatory, and organizational standards.
- Ensure compliance with healthcare laws, regulations, and professional standards.
Qualifications:
· Current registration as a Nurse Practitioner with the College of Nurses on Ontario.
· Five years of experience as a primary healthcare nurse practitioner is preferred.
Proven leadership abilities with excellent oral, written and interpersonal skills
· Computer literacy, experience working with EMR’s
· Current membership in the Nurse Practitioners Association of Ontario and the Registered Nurses Association of Ontario.
· Knowledgeable regarding the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care.
· A well-defined sense of diplomacy, including solid negotiation, conflict resolution, and people management skills.
