Position Profile

Title: Client Care Coordinator

Under the supervision of: Branch Manager

This position is responsible for scheduling home care staff to ensure the delivery of safe, competent care for clients.

Main Duties

  • Coordinates client care requirements by scheduling a caregiver whose qualifications best match the client's requirements based on criteria contained in the referral
  • Obtains and updates employee availability and provides equitable distribution of work while ensuring adequate coverage at all times
  • Responds to client/caregiver/employee inquiries and/or problems immediately, and as appropriate refers to the Branch Manager for necessary follow-up and action
  • Ensures confidential client information is accurate and current
  • Participates in continuous quality improvements in all aspect of the position
  • Strives to continually increase the quality of services provided
  • Strives to continually increase the quality of services provided
  • Performs other duties as required

Skills & Requirements


  • High school diploma


  • Two years scheduling experience
  • Previous experience working in home care would be an asset
  • Strong computer skills

  • Communication skills (bilingualism would be an asset)

Physical Conditions and Capacities:

  • Sitting for extended periods of time

Working Environment:

  • On-call rotation as required to deal with inquiries after hours
  • Dealing with stressful situations

Regional/Provincial Requirements:

  • First Aid/CPR
