Seniors Community Programs

Reporting to the Adult Day Program Manager, the Supervisor is responsible to plan, implement, and evaluate all Adult Day services in accordance with accountability agreements, policies, and approved budget. Working within a collaborative framework will develop strong working relationships internal and external to the program. Responsible for day to day activity including: human resource, quality improvement, and financial management, legislative compliance, and staff training support as directed/delegated by the Manager.
Post-secondary diploma/degree in a health-related discipline, social sciences, gerontology
Bachelor degree in Health sciences, gerontology or related discipline is preferred.
3 years general experience in community service delivery, program development, & community development and engagement activities.
2 years of supervisory experience and leading diversified classifications of frontline staff.
Extensive knowledge of specialized geriatric services, dementia care, stroke, and support services for seniors across the HNHB LHIN.
Must be fluent in French (both written and verbal) for French Language designated programs
Educational training in Dementia studies, Geriatric Mental Health is preferred.
Supervise and coordinate all aspects of assigned program(s) including: (40% of time)
Developing, implementing and evaluating processes to support the daily operations and as defined in the MOH accountability agreements
Provide direction, support, resources, feedback and supervision to assigned staff
Conduct risk management activities to identify and reduce program risks
Audit data entries for integrity issues, provide staff with re-training as required adhering to established work instructions, and submit valid reports to the manager as per schedule for OHRS and quality improvement reporting.
Administers annual Operating budgets for which they are delegated authority ensuring support of Council’s objectives, financial transparency and accountability, monitoring budget adherence, identifying and explaining variances, and financial reporting is effectively managed in compliance with corporate financial policies. Administer and approve the acquisition of goods and services for their direct reports in accordance with the procurement policy
Verify, and approve payroll for direct reports, for Program Manager sign-off.
Provide leadership and support to program staff by (30% of time)
Provide input to and update training resources in collaboration with central support staff
Provide specific training in key identified areas – philosophy, effective relationship building, emergency response, identification of risk, client safe handling, medication prompts and other program specific work processes.
Coordinate and monitor program activity/volumes consistent with defined performance targets; reporting variance to the Program Manager
Monitor and analyze staff activity against standard operating expectations, and performance measures, identify gaps, and develop action plan to support desired outcomes.
Assist with problem resolution escalating unresolved issues to the Manager.
Maintain and review: (10% of time)
Program policies and procedures, work instructions associated with assigned programs, updating as required.
All staff activity and performance within context of program requirements, outcome measures
Triage referrals using a consistent criteria and assign to applicable program interagency team.
Build Strategic Alliances and partnerships to support on-going referrals to services: (10% of time)
Participate in community presentations to key stakeholder groups ensuring collaboration with SCP central office for integration opportunities.
Identify opportunities to the program manager for collaboration between external partners to improve service transitions between shared clients.
Represent community programs at community and public meetings to enhance public awareness of services available.
As directed by the Manager participate in program assessments/ evaluations, partnership tables; bringing content knowledge to inform directional planning.
Supervises staff, coordinating and planning work, providing work direction, assigning tasks and projects, determining methods and procedures to be used, monitoring quality of work, resolving problems and ensuring assigned results are achieved. (10% of time)
Enables results with the organization’s human capital strategy to foster employee engagement.
In conjunction with the Manager provides training and development, coaching and discipline when necessary, and manages staff recruitment.
Ensures focus is directed to service excellence, communication/transparency, innovation, data integrity and work flow integration.
Ensures staff has the information and resources to make successful plans and decisions.
Ensures all people related issues, including recruitment, grievances and labour relations issues, are aligned to HR and Corporate standards and practices.
Helps to break down barriers to employee success, ensuring collaboration and cooperation with other teams within their division and department
Ensures Occupational Health & Safety policies, programs and practices are implemented, and maintained. This includes workplace inspections, monitoring, accident reporting and investigations, and ensuring any observed hazards or lapses in the functioning of OH&S processes, and other OH&S concerns are responded to promptly.
Ensures all individuals under supervision have been informed of hazards and instructed on the necessary risk control and emergency response measures.
In accordance with the Corporate Criminal Record Check Policy, the position requires the incumbent to undergo a Criminal Records Check and submit a Canadian Police Clearance Certificate.
Must maintain ability to travel in a timely manner to other offices, work locations or sites as authorized by the Corporation for business reasons.
