Job Summary
Description: The supported Living Residence program are units designed to provide stable, therapeutic, and supportive living to individuals who are working towards greater independence and wellness.
Position Profile: The support Worker will provide structured support, individualized case management, along with community-based programming to foster skill building, recovery, and social connection. The philosophy of building on strengths and supporting challenges.
The Support Worker must possess excellent communications skills and must be able to work collaboratively with other professionals to provide holistic and cultural care to clients.
Position Reporting: This position reports to the Director, Housing Department
Major Duties and responsibilities:
I. Provide support and guidance to clients with care, compassion and with a non-judgement manner.
II. Conduct a needs assessment to determine the level of programming such as daily life skills, community activities and transportation.
III. Develop and implement case management plans for clients, including crisis intervention, individual and group counselling, and referral to other resources.
IV. Knowledge of trauma informed care and therapeutic practices.
V. Collaborate with other professionals to ensure clients receive comprehensive support.
VI. Advocate for rights of the clients and liaise with relevant agencies.
VII. Keep accurate and up-to-date records of client contract, support and action plans, and progress using case management techniques.
VIII. Foster a team approach with other staff and community agencies.
IX. Provide one-on-one support to clients to access services and resources within the community to foster skill building, recovery and social connection.
X. Support to clients in setting healthy goals, boundaries and life skills through identifying strengths and interests.
XI. Assisting in decision making that aligns with client short-term action plans.
XII. Provide travel when required for programming and appointments.
XIII. Oversee and support medication administration.
XIV. Other duties as assigned by Director, Housing Department.

QUALIFICATIONS;
· Bachelor’s degree in social work or related field of study and/or suitable combination of education and extensive lived experience.
· Valid trauma Informed Care, Applied Suicide Interventions Skills (ASIST), First Aid and CPR, Mental health First Aid, Non-Violent Crisis Intervention, medications Administration, Food Safe, and WHMS Certificates.
· Ability to speak Metis or First Nations language is an asset.
· Knowledge and understanding of the history of Aboriginal peoples and their socio-economic backgrounds.
· Ability maintain confidentiality and respect the privacy of clients.
· Emotional resilience to handle the challenging nature of the work and maintain a self-car plan
· Excellent communications sills, verbal and written to effectively communicate with clients.
· Awareness of the services and resources available within the community of Saskatoon.
· Must be comfortable working with and providing intensive supports to individuals. Willing available to work non-traditional hours such as evenings and weekends.
· Valid Class 5 driver’s license with reliable transportation and must e willing to obtain Automobile Insurance and Coverage.
· Must have a working knowledge of windows-based computer software programs such as Microsoft Word and Excel
· Criminal and Vulnerable Persons Record Check.
Pay: $19.00-$24.00 per hour
