The Community Outreach Worker supports clients facing barriers to health and social services through community outreach, advocacy, system navigation, and health promotion. Working within Quest CHC’s interdisciplinary team, the role connects clients to primary care and community resources using trauma-informed, harm reduction, and culturally safe approaches. The position also supports care coordination, community partnerships, and initiatives that reduce health inequities.
QUALIFICATIONS INCLUDE:
- Successful completion of a college or university program in social work, child and youth care, counseling or other social science or health-related discipline.
- 2 to 4 years experience in a community-based social service setting
- 2 to 4 years providing assessment and brief intervention counselling is an asset.
