Contact Tracers will be responsible for identifying members of Unity Health exposed to patients or staff diagnosed with infectious diseases. They will investigate, collect and record information as per established guidelines, protocols and standard documentation practices. In addition, they will perform work in accordance with municipal, provincial and federal guidelines and directives related to COVID-19 pandemic.
Engage and gather pertinent data on exposure history;
Conduct an exposure risk assessment and clinical assessment to determine risk profile and appropriate work restrictions, testing and isolation requirements;
Develop and implement a comprehensive line list of potential close contacts .
Identify needs goals and concerns regarding provision of care and adjustment to illness;
Develop a care plan that aligns to the individuals goals and needs, as well as, organizational policies and practices;
Interpret diagnostic testing in clinical decision making;
Identify pertinent resources needed to address these current and changing needs;
Identify high need and high-risk staff and prioritizes accordingly;
Formulate comprehensive assessment, intervention and continuing care and return to work plans;
Draws on professional training to apply clinical expertise, skills, knowledge, judgment and theoretical knowledge in formulating a care plan.
Provide clinical advise and use clinical knowledge, judgement and expertise to provide direction to the contact.
Maintain accurate documentation and handover communication
Use sound judgement and clinical knowledge to escalate cases, as appropriate.
Work with the above stakeholders to manage suspect and outbreak procedures
Consult formally and informally with members of the interdisciplinary team both within the organization and with external community partners such as Toronto, Public Health
Attend team rounds to provide input: screen for staff that may be at risk in their efforts to adapt to illness, medical treatment and/or life transitions, to provide input regarding identified needs, wishes and concerns of staff and potentially their family members and to seek/receive updates on medical/functional needs of the staff member;
Organize and facilitate meetings as required;
Initiate consultation/collaboration on issues such as of informed consent and confidentiality;
Able to work effectively as a member of an interprofessional team.
Continually modify the intervention/plan to be responsive to the needs of patients/families in the organization.
Use sound judgment and leadership skills to escalate issues in an appropriate and timeline manner to mitigate risk to the individual, staff and patients, and the organization
Maintain daily contact with the team lead providing status updates and escalating issues or identified needs in a timely manner.
Be able to modify and adapt methods of communication to account for the differing needs of contacts especially in stressful and difficult situations and ensure polite, efficient and supportive communication exists at all times.
Accurate data must be collected, maintained and validated to support efficient and effective decision making at the senior leadership level.
Work in collaboration with Infection, Prevention and Control team members and clinical program leadership to identify Unity Health workers who may have been exposed to to a Covid-19 patient, staff or community member
Regulated health professional in good standing with relevant regulatory body.
Undergraduate degree required, in a health discipline preferred.
Demonstrated ability to work collaboratively within an interprofessional team, as well as independently.
