Location: Rotating within various locations within East Toronto but not limited to Health Access Thorncliffe Park (45 Overlea Blvd), Access Clinics within the community (10 Gateway Blvd) and Michael Garron Hospital (825 Coxwell Ave)
Background:
Heath Access Thorncliffe Park (HATP) is committed to providing coordinated Health & Wellness services to the Thorncliffe Park community. It is a community driven initiative aimed to enhance interdisciplinary comprehensive primary health care, improve access to health, social and community services in Thorncliffe Park. HATP is an innovative partnership between Flemingdon Health Centre (FHC) and TNO-The Neighbourhood Organization (TNO) funded by the Ontario Health- Toronto region. This position is part of the HATP collaboration, is employed by TNO, and works within the Community Engagement team.
TNO is a not-for-profit community based multicultural and multi-service agency, located in Toronto, Ontario, with a vision of neighbourhoods where everyone belongs and succeeds.
Position Summary:
The role involves delivering culturally sensitive services, including client intake, waitlist management, and orientation. Key duties include maintaining documentation, triaging calls, coordinating referrals, and collaborating with service providers to streamline care. The position also supports clients through personalized plans, crisis counseling, and community connections, while contributing to outreach, advocacy, and local community events.
Main Tasks and Responsibilities
· Provide culturally sensitive services, triage client and provider inquiries, and maintain accurate documentation in HATP’s systems.
· Serve as a single point of access for HATP, offering information about the model of care and coordinating with various health and community service providers.
· Conduct intake assessments, manage the client waiting list, and orient new clients to HATP/TNO and relevant community services.
· Identify and address social and health-related needs of clients through navigation, referrals, personalized support plans, and social prescribing.
· Connect clients with community-based programs and services, offering brief crisis support and collaborating with service providers to ensure coordinated care.
· Develop and implement outreach strategies at assigned locations and promote programs in collaboration with internal and external partners.
· Engage in community development initiatives, attend local events, and participate in networks to strengthen community engagement and advocacy.
Skills and Qualifications
· Bachelor of Social Work (BSW) or equivalent, with 1–3 years of experience providing frontline services and outreach to newcomers and equity-deserving communities.
· Strong analytical, crisis intervention, and decision-making skills to assess client needs and respond effectively.
· Proficient in computer use, record keeping, and creating presentations and promotional materials; comfortable with standard office equipment.
· Excellent communication, organizational, and interpersonal skills; able to work independently and collaboratively in diverse, high-pressure environments.
· Demonstrated cultural sensitivity and empathy; willing to undergo training, complete a criminal record check, conduct home visits, and work flexible hours and from various sites based on program needs.
· Ability to communicate in a community-based language is an asset.
