Job Summary
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. This role involves collaborating with physicians and other healthcare professionals to deliver high-quality medical services, manage acute and chronic conditions, and support patient wellness. Experience with medical terminology, medication administration, and electronic health records such as Epic and Cerner is essential. The Nurse Practitioner will play a vital role in improving patient outcomes through expert clinical assessment, treatment planning, and case management.
Duties
- Conduct thorough patient assessments, including history-taking and physical examinations across various age groups.
- Diagnose illnesses and develop individualized treatment plans in collaboration with physicians.
- Administer medications, vaccinations, and other treatments following established protocols.
- Manage acute care cases, chronic conditions, pain management strategies, and dementia or senior care needs.
- Document patient interactions accurately using electronic health record systems like Epic and Cerner.
- Provide patient education on health maintenance, medication use, and disease prevention.
- Coordinate with case managers to ensure comprehensive care delivery in settings such as nursing homes, assisted living facilities, or home care environments.
- Support patients with complex needs including pediatrics, dementia care, or home-based services.
- Maintain compliance with healthcare regulations and safety standards while delivering compassionate patient-centered care.
Qualifications
- Valid Nurse Practitioner license with current certification to practice independently.
- Familiarity with electronic medical records systems such as Epic and Cerner.
- Excellent communication skills to effectively collaborate with multidisciplinary teams and educate patients.

Pay: $300,000.00 per year
