Title : Case Worker

Overview: The John Howard Society of PEI is seeking a Case Worker for their WrapAround Rural Housing Program. The Case Worker is responsible for the provision of service delivery for those individuals/ families facing homelessness or the risk of homelessness in the rural areas of Prince Edward Island (excluding West Prince).

Reports to Program Manager

Key Duties/ Responsibilities:

Intake- accept self- referrals from clientele as well as community based and government services

Assessment- identify housing concerns, needs and wants of the client

Advocate and clarify processes and procedures for clients in regards to community and government agencies

Case plan- create an individual case plan for clients which reflects the needs and wishes of the client

Partnership- working with various community and gove4rnment agencies to organize support

Provide a support network to assist in finding and maintaining safe and secure housing

Provide services including daily coping skills, anger management, recovery and support

Connect individuals to resources in their home communities to address their housing needs and enhance their personal support networks

Monitor client and provide intervention as needed

Participate in case conferences with community agencies on behalf of the client

Provide on-going in-home site support on a weekly basis as required for housing retention

Key Skills Required:

Experience with “at risk” population

Proven ability to organize, prioritize and multi-task

Practical and effective problem solver

Works well under pressure while maintaining a positive attitude

Works well independently as well as on a team


Strong communication skills both written and oral

Excellent interpersonal skills

Punctual and reliable

Experience with a variety of software programs such as Groupwise and Microsoft Word


Post-secondary education in the field of Human Services

Experience working with individuals experiencing mental health issues and/or addictions

ASSIST training would be an asset

Ability to advocate effectively on behalf of clients

Understand and practice professional boundaries

Criminal Record check is required

Reliable transportation

A valid Prince Edward Island driver’s license and a vehicle with adequate insurance coverage
