The Dr. Peter Centre provides compassionate HIV care for people living with significant health issues including mental illness, trauma, substance use, unstable housing and poverty.
Through its day health program, 24-hour care residence and enhanced supportive housing program, the Dr. Peter Centre provides healthy meals, counselling, therapies, nursing, and a safe place for peer socialization and support. This integrated model of care successfully engages individuals in their health care, improving adherence to HIV treatment and overall health.
We are currently looking for a Nurse Practitioner, to join our dynamic teams on a permanent or 2-year locum full time basis.
Position Summary:
Reporting to the Director, Campus of Care, the Nurse Practitioner provides shared and primary care to adults with HIV and complex health issues including substance use in the Day Health and Residential program at the Dr. Peter Centre.
The role of the Nurse Practitioner at the Dr. Peter Centre is dynamic and innovative and encompasses providing point of care primary care, in addition to secondary and tertiary preventative care with the overall goal of improving the person’s health. Using an interdisciplinary approach to care, the Nurse Practitioner is responsible for the comprehensive assessment of participants utilizing our clinical services. The Nurse Practitioner initiates treatment including health care management, therapeutic interventions and prescribes medications in accordance with the statutory and regulatory standards, limits and conditions, and employer’s policies and procedures. The Nurse Practitioner provides professional guidance to nursing care staff and collaborates with the participants’ primary care providers. In our Day Health Program, the Nurse Practitioner is responsible for comprehensive assessment, and treatment and primary care of HIV, Hepatitis C, and Substance Use Disorder including OAT, iOAT and MAP. In our Residence program, the Nurse Practitioner works in a shared care program with the residences’ primary care provider and the nursing team on the units. The Residence has a mix of long-term beds and 90-day stabilization beds.
The Nurse Practitioner works independently and as part of an inter-professional team of nurses, dietician, counselors, therapists and consulting physicians, as well as collaboratively with community primary care physicians, specialists, rehabilitation clinicians and wound care clinicians.
The Nurse Practitioner is a practice leader assisting developing, implementing and evaluating of evidence-based activities that influence the health service and practices.
Key Responsibilities:
- Diagnose and treat previously undiagnosed participants for undifferentiated disease and conditions within the Nurse Practitioners’ scope of practice;
- Write orders for treatment and medications;
- In collaboration with primary care physician, monitors ongoing care, orders appropriate screening diagnostic investigations; interprets reports of investigations and analyzes information to monitor progress and plan treatment;
- Initiate treatment including health care management, therapeutic interventions and prescribe medications in accordance with the statutory and regulatory standards, limits, conditions, and employer’s policies and procedures;
- Provide professional mentorship to other health providers and practice autonomously and interdependently within the context of an interdisciplinary health care team;
- Participate in the development of policies, procedures and protocols relevant to nursing, interdisciplinary care, and health system practices;
- Participates in staff education and capacity building;
- Opportunity to participate in research and pilot projects;
- Participate in quality improvement and program evaluation including self and peer review processes;
- Responsible for own professional development and maintaining competence in the position and maintaining collaborative practice with the program-specific consulting physicians
- Participate in the orientation of new staff and the support of students; and
- Perform other related duties as assigned or required.
Education, Qualifications and Experience:
The preferred education, qualifications and experience of the Nurse Practitioner are:
- Current registration as a Nurse Practitioner with the BC College of Nurses and Midwives;
- Recent and relevant primary care experience treating adults;
- Preferred knowledge and experience in the following areas: HIV, Hepatitis C; substance use disorder and end of life care;
- Ability to communicate with clients about health findings, diagnosis, treatment, and self-care;
- Ability to collaborate, consult, and refer clients to physicians and other healthcare providers;
- Ability to assess and recognize population health trends;
- Ability to plan and implement strategies for population-based prevention and health promotion;
- Ability to identify and respond appropriately to legal and ethical issues that may arise in client care; and
- Ability to self-assess performance and assume responsibility and accountability for own professional development, educational, or consultative assistance when appropriate.
General Working Conditions:
This is a permanent or 2-year locum full time position. This role requires flexibility, the ability to prioritize workloads, and adaptability in a fast-paced environment with shifting demands and deadlines. The position involves supporting multiple operational functions simultaneously and handling unexpected tasks on short notice.
