Overview
The Indigenous Transitions Facilitator (ITF) focuses on providing care to clients and communities while upholding the mission, vision, and values of Independent First Nations Alliance (IFNA) Health Services. This position supports and advocates for indigenous patients throughout their healthcare journey, ensuring culturally sensitive and holistic care. This position plays a critical role in supporting patients transitioning from hospital care back to their communities through coordination of discharge plans that ensure seamless transitions and providing culturally sensitive support throughout the process.
Responsibilities
- Coordinate client-centered care by collaborating with healthcare providers, referral partners, and community services to ensure seamless care transitions.
- Schedule home visits, client conferences, and virtual care appointments as required.
- Support the delivery of remote healthcare services, including telemedicine, to improve access for Indigenous communities.
- Advocate for Indigenous clients by promoting culturally safe care and ensuring cultural and spiritual needs are respected.
- Collaborate with clients, families, healthcare teams, and community partners to develop comprehensive discharge plans.
- Connect clients with appropriate health, social, and cultural supports to promote successful transitions back to their communities.
- Facilitate communication among all partners to ensure continuity of care and ongoing access to services.
- Problem-solving, good judgement and decision-making skill and ability to stay calm and composed especially in times of crisis or urgencies.
- Strong interpersonal and collaboration skills with the ability to build trust and rapport with patients, families, healthcare teams, and community stakeholders.
- Empathy, compassion, and cultural sensitivity to address the unique needs and challenges faced by indigenous patients during their transition from hospital to community care.
- Understanding of Indigenous history, traditions, and beliefs with the ability to implement cultural values and practices within the delivery of services; Ability to speak one of the Indigenous dialects is an asset.
- Familiarity of Indigenous communities and organizations; experience working for an Indigenous organization.

Salary Range: $71,330-$ 81,521
