Job Summary:

As a Nurse Practitioner, you will work collaboratively with our medical team to deliver exceptional patient care and support. Duties and Responsibilities:
- Completes comprehensive health history and physical assessment for identified residents.
- Identifies and orders diagnostic tests and procedure according to approved medical directives within the scope of practice.
- Collects, interprets and analyzes data from multiple sources and uses this data to formulate medical diagnosis within scope of practice
- Works collaboratively with residents, physicians and other members of the interdisciplinary team to develop a plan of care, and revise as required based on resident and family engagement/response.
- Prescribes pharmacological and non-pharmacological therapies according to approved medical directives
- Perform skills and invasive/non-invasive procedure related to specialty
- Initiates interventions for the purpose of stabilizing residents in urgent and emergent situations.
- Liaises, refers and advocates with community resources to coordinate admissions, transfers, and departures/deaths: hospitals (acute care, rehabilitation, and palliation), public health department, home care program, long term care facilities, ambulance services, diagnostic services, the coroner’s office and social work services
- Acts as a coordinator of care for complex or acute residents, sharing nursing expertise through health teaching and demonstrating leadership while working in collaboration with other care providers
- Monitors staff performance and the environment to ensuring safe workplace for staff; provides education where needed
- Performs other related duties as required and assigned
- All duties and responsibilities in accordance with the College of Nurses Nurse Practitioner Standards
Knowledge, Skill and Experience:
- Advanced nursing skill / training in specialized areas such as IV and / or wound care is an asset

- Excellent reading, writing and organizational skills
Education:
- Current registration in the Extended Class with the College of Nurses of Ontario required
- Master’s Degree in Nursing required
- Minimum 3 years Nursing experience, with Geriatric expertise preferred
- CAN Specialty Certification in Gerontology preferred
- CPR (requested every other year for competency)
Hours:
Pay: $60.66-$75.83 per hour
