Department: Mental Health Hospital to Home
The Case Worker through the Hospital to Home Program – Mental Health (H2H MH) will support stabilization after hospitalization and optimize recovery while being connected to other community supports. H2H MH will provide care coordination and oversight to those receiving service from the program. H2H MH services are individually tailored with each client and address the preferences and identified goals of each client.
QUALIFICATIONS
Certification/Experience/Knowledge
- Must have minimum two year diploma or degree in the health/social sciences required
- Recent 2-3 year experience working in the field of Addictions and Mental Health required
- Recent 1-2 year experience working in Community Mental Health and Addictions required
- Current Basic Life Support (BLS) for Health Care Providers (HCP) Certificate required (recent within the last twelve months)
- Current Gentle Persuasive Approach (GPA) Certificate an asset
Skills/Abilities:
- Knowledge of Best Practices in mental health and addictions
- Demonstrated knowledge of and skill in applying the principles of psychosocial rehabilitation (including assessment, treatment and rehabilitation)
- Demonstrated knowledge and experience in implementing recovery informed approaches including motivational interviewing and Cognitive Behavioral Therapy (CBT) informed techniques
- Previous experience using different evidence based therapeutic interventions
- Advanced knowledge of community resources and service systems that support individuals with serious mental illness and concurrent disorders
- Advanced skills in case management, system navigation and advocacy to support individuals in accessing resources and services
- Conduct community/home visits to support with transitions and stabilization after hospitalization and optimize recovery while being connected to other community supports

- Ability to work collaboratively with all members of an Interdisciplinary health care team which includes clients, families and community partners
- Demonstrated ability to work independently and in a self-directed manner

- Ability to provide a safe and non-threatening environment to clients
- Excellent crisis intervention assessment and problem solving skills
- Valid class G Driver’s License and Ministry of Transportation Driver’s Abstract required for extensive travel within the catchment area
- Access to a safe and reliable vehicle with sufficient insurance ($2,000,000) to allow for work-related use
- Demonstrate excellent interpersonal and communication skills
- Good attendance and work record required
- Ability to meet the physical demands required for a community based team
- Must support and contribute to a culture of safety and prevention of adverse health events in this organization
- Must demonstrate good attendance and work record
- Bilingualism (Advanced level English/French) is an asset
