THE OPPORTUNITY - Housing Services Practitioner - Complex Medical Needs
WHAT YOU WILL DO
- Support individuals to know their rights, voice their thoughts and needs, and advocate for the services they require
- Assist individuals in developing social skills, building meaningful relationships, and becoming valued and included members of their community
- Provide person-centered support that recognizes each individual’s unique strengths, goals, and challenges
- Identify behaviours of concern and contribute to the development, review, and monitoring of plans that help individuals manage behaviours or situations of concern
- Assist or provide full support in personal care routines such as bathing, dressing, and grooming
- Assist or provide full support to individuals in maintaining their health, including attending medical appointments, administering medication, liaising with pharmacies, supporting emotional well-being, and promoting healthy lifestyles
- Provide comprehensive support in home maintenance and daily living activities, including meal preparation, grocery shopping, cleaning, and laundry — particularly in homes with complex medical needs
- Support individuals in managing their personal affairs such as budgeting, banking, and paying bills
- Demonstrate a high standard of professionalism aligned with Vecova values
- Maintain a positive working relationship with all Vecova team members
- Represent Vecova in a professional manner
- Demonstrate a high level of initiative and enthusiasm
- Maintain confidentiality at all times
- Demonstrate a high level of administrative and IT capacity
WHAT YOU BRING
We are looking to engage team members with diverse skills, experiences and interests. These are some of the competencies and credentials we are looking for:
- Diploma and/or Degree in Community Rehabilitation, Disability Studies or a related social science
- Minimum of two years of experience working with individuals with disabilities or an equivalent combination of education and directly related experience
- Comfort with consistent engagement with individuals, including those who do not use words to communicate
- Well-developed and effective communication skills (written and oral)
- Demonstrated skill in critical thinking
- Demonstrate a high standard of professionalism aligned with Vecova values
- Maintain a positive working relationship with all Vecova team members
- Represent Vecova in a professional manner
- Demonstrate a high level of initiative and enthusiasm
- Maintain confidentiality at all times
