Position Overview
Summerville Family Health Team is looking for a highly skilled, experienced and enthusiastic Adult Social Worker to provide direct service, outreach, and case management to Summerville patients. Reporting to the Manager, Programs and Community Outreach and working alongside an interdisciplinary team, the Adult Social Worker will provide family-centered counseling to individuals, families, and groups while also helping in navigating community resources. Additionally, the social worker will be responsible for the development, management, and implementation of programs and partnerships that provide support and treatment to Summerville patients.
Duties and Responsibilities:
· Develops short-term therapeutic relationships with patients and/or families to foster coping and adaptation of health promoting strategies or chronic conditions to help maintain or improve quality of life;
· Conduct psychosocial assessments to provide patients and families with coping abilities and support networks, identify needs and concerns, integrating physical and environmental factors;
· Develop and/or adapt, manage, implement and evaluate psycho-educational programs, independently or in collaboration with the interdisciplinary team, related to the prevention and management of mental health issues;
· Provide assistance in linking patients with community and internal resources, as part of a multi-disciplinary plan of care;
· Maintain accurate, concise and confidential EMR documentation where applicable, pertaining to patient history and encounter data;
Required Education, Skills and Qualifications
· Masters of Social Work degree from a post-secondary institution; (we do not accept Bachelors of Social Work or other degrees that are not a Masters of Social Work)
· Registered member in good standing with the Ontario College of Social Workers and Social Service Workers;
· Minimum of 3 years related experience in a health care environment and/or within the mental health sector.
Additional Skills
· Able to partner and work collaboratively as part of a multi-disciplinary team;
· Excellent written and verbal communication skills, including facilitation skills;
· Good critical thinking and decision-making skills;
· Knowledge of Microsoft Word, Excel and EMR (e.g. Accuro).

Pay: $33.19-$39.05 per hour
