About FRAFCA:

Fraser Region Aboriginal Friendship Centre Association is a leading provider of after-school, summer, and college programs for over 3,000 students in the Surrey, BC area. We are also a provider of transportation and special education services for over 300 students with disabilities in the Surrey, BC area.

Position Overview: (Non Union)

In accordance with the established vision and values of FRAFCA and in collaboration with the multidisciplinary team, performs assessments, plans, implements and provides personal and nursing care to patients/residents/clients. Administers oral, subcutaneous, and intramuscular medications in accordance with pharmaceutical and organizational policies and procedures.

The scope or extent of the skills that the NP performs varies according to the predictability of the outcome for a given client population. The NP operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM), and according to organizational policies, procedures, standards, and unit-specific protocols.

Job Description:

Role and Responsibilities:

Assesses and monitors patient/resident/client/family’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs. Where appropriate, encourages patient/resident/client participation in activities of daily living. Establishes and maintains a therapeutic relationship with the patient/resident/client and significant others through the use of interpersonal and interviewing techniques ensuring patient/resident/client choice and autonomy in decision-making and care planning.
Contributes to the development and ongoing changes to the individualized plan of care, including attending and providing input into multidisciplinary care planning meetings and family conferences. Advocates for the interest of the patient/resident/client and significant others.
Administers oral, subcutaneous, intravenous, and intramuscular medications including PRN medications in accordance with organizational and pharmaceutical policies and procedures.
Provides maintenance of peripheral intravenous infusions by assessing the intracath insertion site, monitoring flow rate and discontinuing peripheral IV access when ordered by the physician.
Performs nursing procedures and interventions and monitors and evaluates the effectiveness of the procedures and interventions. Procedures include head to toe assessments, airway management, nutrition management, infusion management, catheterizations, applying and changing simple sterile and/or non-sterile dressings, colostomy care and irrigation, taking/recording blood pressures, administering suppositories and enemas, taking/recording temperature, pulse and respiration; records observations and reports problems and/or changes to designated nursing staff. Performs CCH specific nursing procedures and interventions as required.
Communicates the client’s condition and changes to the designated RN/RPN.
Provides personal care to patient/resident/client including assisting with bathing, dressing, care of skin and hair; changing beds; assisting with toilet needs; and overseeing exercise routines.
Assists patient/resident/client with meals by serving and collecting meal trays and feeding designated patients, maintaining intake and output records as required.
Identifies and assesses potential emergency situations and develops short-term strategies to handle appropriately, including actions such as applying suicide alertness and prevention skills, responding to overdose in accordance with established procedures, using first aid, assisting a client to access drug checking services, and assisting a client to access overdose prevention services.
Performs admissions, evictions, and transfers of patients/residents/clients according to facility operating policies and procedures.
Participates in “on-call” and “call-out” schedules to enhance and extend support activities for residents and families in the Complex Care Housing program.
Documents and updates all relevant information about care and procedures to achieve patient/ resident/client safety and comfort according to CCH policy and procedure.
Performs related clerical duties such as answering the telephone, making appointments, ordering supplies and initiating diagnostic requisitions.
Assists in the orientation and sharing information with new staff, students, and other members of the multidisciplinary team.
Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings and committees.
Maintains a safe environment for patient/resident/client, significant others and staff by adhering to established safety policies, standards and procedures including emergency procedures and infection control guidelines. Reports unsafe conditions and malfunctioning equipment.
Performs other related duties as assigned.

QUALIFICATIONS:

Education, Training and Experience:

Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience. Current full practicing license with the British Columbia College of Nurses and Midwives (BCCNM). Current CPR certification required.

Food Safe Level 1 Certificate, Basic First Aid Certificate, Non-violent crisis intervention training, and Valid BC Driver’s License.

Skills and Abilities:

Ability to communicate effectively, both verbally and in writing.
Ability to deal with others effectively.
Ability to organize work.
Ability to operate related equipment.
Physical ability to carry out the duties of the position.
