The STAR Family Health Team delivers high-quality, comprehensive primary care through two clinics in Stratford and one in Tavistock. Our collaborative team includes family physicians, nurse practitioners, nurses, mental health professionals, dietitians, pharmacists, and a dedicated administrative staff. Together, we create a supportive, innovative, and patient-centered environment.

We are seeking a Permanent Nurse Practitioner (4 days per week) who thrives in a team-based setting and is passionate about making a meaningful impact in the lives of patients.

What You’ll Do

Provide comprehensive primary care, including acute, episodic, and chronic disease management.
Collaborate with physicians, nurses, and allied health professionals to deliver integrated, patient-centered care.
Contribute to program development, resource creation, and community partnerships within the Family Health Team.
Utilize your clinical expertise and procedural skills (experience in procedures considered an asset).

What We’re Looking For

Current registration with the College of Nurses of Ontario (RN-EC) and professional liability insurance.
Minimum of 3-5 years relevant nursing experience required.
Strong clinical judgment, excellent communication, and a collaborative spirit.
Comfort working in a fully computerized environment, with proficiency in Microsoft Office and EMRs on both Apple and PC systems.
A motivated, self-directed professional who thrives in a busy clinic environment and demonstrates flexibility, organization, and teamwork.
