The Patient Coordinator (PC) is a trained Community Paramedic (CP) who is provided with extra training to work within the Mobile Integrated Healthcare- Coordination Center (M.I.H.C.C.). The Patient Coordinator (PC) will be responsible for a myriad of tasks and duties directly related to the Mobile Integrated Healthcare- Coordination Center (M.I.H.C.C.). The Patient Coordinator (PC) will work under limited guidance and supervision and will provide guidance and support to Community Paramedics (CP), E.M.S. crews, and regulated community healthcare providers. The goal of M.I.H.C.C. is to ensure that patients receive the right care, at the right time, by the right provider and are linked with appropriate resources to avoid reliance on the acute care system. The Patient Coordinator (PC) will be expected to make informed decisions and utilize clinical judgment when speaking with E.M.S. crews and other healthcare practitioners. The Patient Coordinator (PC) will be required to manage and appropriately triage emergency situations from callers and transfer information to 911. Candidates will be rotated through a dedicated Community Paramedic unit 25-50% of their rotation. This ensures that clinical competency and skills are maintained. A Community Paramedic (CP) will provide immediate or scheduled medical supervision and treatment(s) not currently in the community setting in collaboration with the patient's existing healthcare services. The Community Paramedic (CP) works collaboratively with other community healthcare services to provide care in place for patients who do not require immediate transport to an acute care setting for medical treatment or assessment. While the Community Paramedic (CP) 's primary role focuses on delivering primary and urgent care, they must still be able to provide emergency treatment should the situation arise.
Description:
As an Advanced Care Paramedic (ACP) with ALTA Paramedic Health (APH), you will assess emergency scenes and patients and may transport ill or injured individuals from emergency scenes and between facilities. This includes operating ambulances or other modes of transportation. You will provide patient care by administering medications, using advanced life support (ALS) airway adjuncts, and delivering electrical therapy. You may also lead the ALS team in providing clinical care during patient transport or at emergency scenes. You will support the readiness of vehicles by ensuring ambulances and other transport units, including non-emergent transport (NET) vehicles, are properly stocked, maintained, and equipped in accordance with APH standards. The working environment for an ACP is demanding and includes exposure to heat, cold, rain, snow, and wind, moving traffic, and industrial and agricultural equipment. You will be required to travel in all weather conditions and to remote locations (e.g. backcountry), with exposure to hazardous and graphic scenes and direct exposure to bodily fluids. This role is also physically demanding. You must be able to endure frequent lifting and carrying of up to 105 lbs, while transporting and transferring patients, supplies, and equipment. You must be able to endure frequent, simultaneous kneeling and trunk flexion for extended periods. Additionally, you must stand and sit for extended periods, as well as bend, twist, side-carry up to 20 lbs in each hand and assume awkward positions.

Required Qualifications:
Non-restricted registration in good standing with the Alberta College of Paramedics (ACOP) as an Advanced Care Paramedic (ACP). Current Basic Cardiac Life Support (BCLS) CPR certification. Must hold, or be eligible to obtain, a valid Class 4 Alberta Operator's Licence (or equivalent), along with a five (5)-year driver abstract dated within six (6) months prior to commencing work, with no more than six (6) demerits. Proof of successful completion of the Functional Abilities Recruitment Evaluation (F.A.R.E.) test, verified by a certificate from Lifemark, is required for candidates who do not currently hold a clinical position within ALTA Paramedic Health (APH) (including VESST, Clinical Educator, EMR, PCP, ACP, and Community Paramedic roles). The F.A.R.E. test must be completed no more than six (6) months prior to the date of application. APH may verify results with Lifemark to confirm successful completion. Additional Required Qualifications:
Advanced Care Paramedics must maintain clinical competencies within their scope of practice as determined by A.H.S. Must be able to aid in patient transfers, support the lifting and movement of patients as needed and assist E.M.S. staff with the loading of patients onto stretchers Be able to demonstrate satisfactory past work performance and relevant work experience. Strong interpersonal communication skills and ability to collaborate in an interdisciplinary team. Excellent ability to multitask utilizing various computer software and technology combinations in a busy and dynamic environment. Completion of an accredited Advanced Care Paramedic program. Active registration with Alberta College of Paramedics (A.C.P.) as an Advanced Care Paramedic. Current Basic Cardiac Life Support - Healthcare Provider (BCLS-HCP). Completion of the Functional Abilities Recruitment Evaluation (F.A.R.E.) test taken no more than six months from the application date.
Preferred Qualifications:
Previous Community Paramedic experience is an asset. Previous Patient Coordinator experience preferred. Previous experience in a community health or urgent care setting is preferred. Previous leadership experience preferred. This posting will be used to fill multiple position.
