about the role:
As part of our MOBILE HEALTH BUS, the Nurse Practitioner (NP) will be an integral part of a team that brings primary care, brief case management and harm reduction services to homeless and precariously housed individuals with complex mental health and substance use issues in the Downtown East community.
Scope of the Mandate:
The Nurse practitioner (NP) will provide a full scope of primary care and liaise as required with other members of the Health Bus team and external health care providers and community partners to ensure the delivery of comprehensive health care to clients on the Health Bus and complete follow up at Sherbourne Health and in the community as required.
The Nurse Practitioner understands the impact of the broad determinants of health on people’s ability to attend to their health care needs and provides care within a health promotion framework. The Nurse Practitioner will work collaboratively within a multidisciplinary team and in keeping with the core objectives and values of Sherbourne Health. This position is responsible for meeting and maintaining the standards of practice prescribed in legislation and regulations that apply to the position as outlined in the practice expectations specific to Registered Nurses in the Extended Class from the College of Nurses of Ontario. In this role, the Nurse Practitioner works to full scope of practice.

Responsibilities and Tasks:
CLIENT SERVICE:
- Provides episodic primary care to clients, taking into account environmental, social, psychological and medical risk factors in a manner that can be understood by clients and is appropriate to their individual situation.
- Discusses health concern/diagnosis with the client and together determines an appropriate plan of care.
- Provides ongoing follow-up and evaluation of the plan of care with the client
- Provides supportive counseling to clients with mental health/emotional concerns and refers to internal or external resources as necessary.
- Collaborates with Health Bus team members and refers clients to other services in the community to ensure clients receive comprehensive, competent and appropriate health care.
- Assesses clients for routine health and illness care and to prescribe treatment.
- Coordinates and conducts sexual health visits, cervical screenings and other visits/ clinics as needed.
- Consults and/or refers for further assessment or treatment for the client when appropriate and/or beyond the Nurse Practitioner scope of practice.
- Prescribes medications and orders diagnostic testing in accordance with the scope of practice for Nurse Practitioners.
- Acts as a health advocate for clients
- Uses health care resources effectively to benefit clients. Delivers episodic and primary care services within the Nurse Practitioner (NP) scope of practice as outlined by the College of Nurses of Ontario (CNO) and Bill 179.
- Refers clients to other primary care services as required and provide follow up.
- Provides primary healthcare grounded by the five principles of Primary Healthcare as outlined by the World Health Organization.
- Works from a harm reduction approach that includes the provision of Naloxone and harm reduction supplies as needed
- Identifies barriers to health and facilitates access to resources.
- Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with SHC related policies, health and safety legislation and best practices
Health Promotion and Education
- Participate in the development, planning and evaluation of the Health Bus program.
- Counsels and educates clients on health related and lifestyle issues.
- Facilitates health promotion programs which promote mental and physical health.
- Acts as resources to staff and the community regarding current trends in health care as well as general health issues.
- Collaborate with community partners to provide education and information regarding improved access to primary health care services
- Participates actively on networks and coalitions dealing with issues affecting primary health care with a focus on homeless under housed populations
- Collaborates and consults with multidisciplinary staff within host organization, partnering organizations and broader community to facilitate comprehensive primary care provision
- Acts as a preceptor to NP and Registered Nursing (RN) students and mentors new staff members and/or students
- Engages in evidenced based practice and ongoing learning
- Collaborates with the multidisciplinary team to assist with quality improvement and development of creative strategies to meet client needs
- Advocates for individual clients need to ensure access to effective care, follow-up and resources from both an individual and systemic level
- Collaborate with colleagues to identify population trends, develop research questions and/or conduct research studies
- Facilitates knowledge translation by educating colleagues on topics related to client care
- Initiates and/or participates in systemic advocacy activities directed at achieving health equity by improving the social determinants of health, eliminating stigma and developing healthy public policy.
Education
- Pursues on-going professional education and development through approved continuing education.
- Completes minimum number of hours continuing education per year as designated by the College of Nurses.
- Participates in the training of other healthcare professional students.
Administrative Duties
- Identifies areas where the development of protocols and procedures are needed to improve client services or to promote more effective staff functioning.
- Participates in meetings with the Health Bus team, the broader organization and partners, as required.
- Maintains accurate records of clients’ visits in the Electronic Medical Record system.
- Records encounters, including telephone conversations, as appropriate for data collection and analysis.
- Assists team in administrative and clinical duties as required.
- Other duties as required.
To thrive in this role, you’ll need
education/experience:
- Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario (CNO) (a generalist Nurse Practitioner holds an Extended Class Certificate of Registration with the College of Nurses of Ontario).
- Current Basic Life Support (BLS) certification.
- Evidence of completion of CNO-approved controlled substances education for Nurse Practitioners.
- A minimum of 2 years of clinical experience as a Nurse Practitioner with a minimum of three years as a Registered Nurse.
- 2-3 years clinical experience in primary care/community settings an asset.
- Clinical experience in providing gender-affirming care for Trans, 2-Spirit and Gender-Diverse clients an asset.
- Demonstrated understanding of working within an anti-oppression framework.
- Experience working with persons marginalized by social location including homelessness, poverty, sexual identity, gender identity, and/or residency status, using trauma-informed, anti-oppressive and harm reduction practices.
- Demonstrated commitment to client-centered, community-based care
- Knowledge and proficiency in current, evidence-based methods and practices in healthcare delivery, with an emphasis on mental health promotion and risk reduction.
- Working knowledge of acts, legislation, and professional standards that govern nursing practice, including the Personal Health Information Protection Act (PHIPA), Regulated health Professions Act (RHPA) and the College of Nurses of Ontario (CNO) Standards and Guidelines.
- Ability to maintain professional practice regarding commitments to colleagues and clients, pertinent legislation, evidence-based standards of care and professional standards of practice.
- Knowledge of the Health Care system, social determinants of health and community resources.
- Ability to work collaboratively within and across teams
- Experience supporting clients to access primary care/community services in Downtown Toronto.
- Experience working with individuals who face mental health and substance use issues
- Knowledge and understanding of primary prevention and health promotion theory and practice.
- Experience and/or strong interest in applying harm reduction principles to reduce negative health consequences associated with drug use among people who use drugs, including opiate agonist therapy (OAT) and safer opioid supply.

- Experience delivering culturally safe care to Black and Indigenous populations is an asset.
- Valid G-Class Driver’s License and Driving Record in good standing to operate the Health Bus required.

working conditions:
- The Nurse Practitioner works primarily on the Sherbourne Mobile Health Bus. Follow-up at Sherbourne Health or other sites in the community when applicable. This role will require work primarily during daytime hours but may require occasional evening and weekends.
