Primary Duties
Utilizes and demonstrates a comprehensive theoretical knowledge base and advanced level of clinical competence in providing care for frail elderly patients in the community. This involves collecting and interpreting data about the health of the client to determine a medical diagnosis and treat complex problems. The role involves being responsible for the care of patients, as MRP, with the support of collaborating physicians.
· Performs assessment activities, including conducting client interviews and comprehensive physical examinations; assesses psychosocial, cultural, and ethnic factors affecting client needs; identifies and orders required diagnostic tests and procedures, within scope of practice and medical directives/protocols and practice guidelines; collects and reviews comprehensive client health data. Additional elements from a comprehensive geriatric assessment may compliment assessments in a standardized format.
· Gathers and interprets assessment and other data (eg. old health records) received from multiple sources; draws a conclusion and determines a medical diagnosis within specialty scope of practice and practice guidelines.
· Identifies expected outcomes derived from the assessment data and diagnosis and individualizes expected outcomes; formulates a comprehensive plan of care.
· Implements the comprehensive plan of care in conjunction with the client that includes prescribing medications and changing dosages, ordering interventions, treatments, and procedures within the scope of NP practice and medical directives; directs the delivery of client care as per the care plan; plays a coordinating role by liaising with other services and team members, as required to coordinate the interdisciplinary plan of care and facilitate the efficient movement of the client through the healthcare system; facilitates communicating the plan of care to the client/family and members of the healthcare team.
· Assesses client/family response to therapy, whether patient attaining expected outcomes, and modifies the plan of care to ensure the highest quality of care has been delivered to clients and their families.
· Documents client history, physical examination data, regular progress notes, referrals as appropriate, MOH forms, insurance forms, team Rounds and family conference notes and other pertinent information in an accurate and timely manner according to organizational policies, procedures, and guidelines.
Education & Experience
- RN with Extended Class Certificate from the College of Nurses of Ontario required
- Recent related clinical experience (3-5 years) preferred
- Current BCLS required
- Demonstrated knowledge and ability to function within the Standards of Practice for NPs as outlined by the Ontario College of Nurses
- Current valid Ontario driver’s license and valid vehicle insurance with 2-million-dollar liability required
- PIECES & U-First Training required
· Preference will be given to applicants with the following qualifications:
- Master of Nursing Degree
- Gerontology Nursing Certificate GNC(C)

- Participate effectively and appropriately in an interprofessional health care team
- Effectively to prevent, negotiate, and resolve interprofessional conflict
- Possesses leadership skills that will contribute to the advancement of clinical practice and the achievement of program goals
Compensation Disclosure: $67.49 to $81.41 per hour
