Position Summary
We are seeking an experienced, compassionate, and patient-centred Nurse Practitioner to join our growing cardiology team on a part-time basis.
The successful candidate will work collaboratively with our cardiologists and allied health team to assess, manage, and follow patients with cardiovascular disease and related medical conditions. This role offers an opportunity to make a meaningful impact on patient outcomes through chronic disease management, cardiovascular risk reduction, patient education, and preventive care.
The Nurse Practitioner will have the opportunity to develop expertise in cardiovascular prevention, hypertension management, lipid management, post-discharge follow-up, and chronic disease management while working alongside a team of experienced cardiologists and healthcare professionals.
We are seeking a confident and independent clinician who is comfortable managing patients within a specialty cardiology setting. Preference will be given to candidates with experience reviewing ECGs and ambulatory cardiac monitoring data, managing medication titration protocols for rate and rhythm control therapies, supporting arrhythmia management, and providing post-procedure triage and follow-up care.
Key Responsibilities
- Conduct comprehensive patient assessments and follow-up visits
- Manage cardiovascular risk factors including hypertension, dyslipidemia, obesity, diabetes, and lifestyle-related conditions
- Monitor and manage patients with stable cardiovascular conditions within a collaborative care model
- Order and interpret laboratory investigations and diagnostic tests within scope of practice
- Develop individualized treatment and management plans
- Provide patient education regarding medications, lifestyle modification, exercise, nutrition, and cardiovascular disease prevention
- Support post-discharge follow-up and chronic disease management initiatives
- Participate in medication titration protocols and cardiovascular risk reduction strategies
- Assist with arrhythmia management and review of ECG and ambulatory monitoring results
- Provide post-procedure triage and follow-up for patients undergoing cardiac investigations and interventions
- Collaborate closely with cardiologists, family physicians, specialists, and allied health professionals
- Maintain accurate and timely clinical documentation within Accuro EMR
- Participate in quality improvement and patient care initiatives
Qualifications & Requirements
- Bachelor of Science in Nursing (BScN) or equivalent degree from an accredited university
- Current registration as a Nurse Practitioner (Primary Health Care or Adult) and in good standing with the College of Nurses of Ontario (CNO)
- Current professional liability protection
- Minimum 3 years of relevant clinical experience, preferably in cardiology, internal medicine, or a related specialty
- Strong clinical assessment, diagnostic reasoning, and patient management skills
- Experience managing chronic cardiovascular conditions, including hypertension, dyslipidemia, arrhythmias, heart failure, and diabetes
- Ability to review and recognize ECG findings and ambulatory cardiac monitoring results and escalate concerns appropriately
- Experience with medication titration protocols, including rate and rhythm control therapies and cardiovascular risk reduction medications
- Comfortable providing post-procedure triage and follow-up for patients undergoing cardiovascular investigations and interventions
- Excellent communication, interpersonal, and patient education skills
- Ability to work independently while collaborating effectively within a multidisciplinary healthcare team
- Proficiency with electronic medical records (EMR); Accuro EMR experience is considered an asset

Pay: $68.00-$74.00 per hour
