PURPOSE: The Busby Centre is a community-based agency that believes in the Housing First and Harm Reduction philosophies and practices. We strive to improve the quality of life for people who are struggling with chronic homelessness, significant mental health issues and substance misuse. Our program provides individuals a place to stay, and staff support 24/7, along with a variety of on-site services to help people experiencing homelessness transition to housing and successfully stay housed.
The Assertive Street Outreach Case Manager builds trusting, professional relationships with individuals experiencing homelessness and/or significant housing precarity. Operating with a Housing First and harm reduction lens, this role is responsible for providing assertive, mobile, person-centered outreach to support service users in accessing shelter, income supports, health services and permanent housing.
Please note: This is a PERMANENT contract, subject to yearly renewal based on employee performance and the availability of sufficient program funding. We are currently hiring THREE (3) FULL-TIME employee for this position.
LOCATION: Barrie/Springwater, South Georgian Bay/Midland
RESPONSIBILITIES:
- Direct Service user Support & Case Management:
- Conduct regular, scheduled street outreach and respond to referrals for unsheltered individuals.
- Provide immediate crisis intervention, wellness checks, and support basic needs (food, clothing, emergency shelter).
- Develop individualized support plans with service users, focusing on stabilization and transition to permanent housing.
- Connect service users with appropriate services including healthcare, addiction treatment, mental health services, and income supports.
- Assist with documentation, applications (ID, OW/ODSP, housing), and advocacy.
- Use a strengths-based, trauma-informed approach to build rapport and promote self-determination.
2. Housing Focused Interventions:
- Identify and assess barriers to housing and actively support housing searches.
- Collaborate with landlords, housing providers, and community partners to secure appropriate housing.
- Provide follow-up support post-housing placement to ensure stability and retention.
3. Documentation and Evaluation:
- Maintain accurate, timely case notes and service user files using organizational and funder-required systems (e.g., HIFIS).
- Track outreach activities and outcomes; contribute to program evaluation and improvement.
4. Community Collaboration:
- Participate in Coordinated Access, By-Name List meetings, and community outreach events.
- Coordinate with police, EMS, shelters, and service providers to support service user safety and care continuity.
- Act as an ambassador for the organization and advocate for the rights and dignity of service users.
5. Cleaning and Inventory Management:
- Uphold health and safety standards by performing daily cleaning and garbage removal within the outreach program environment (workspaces, including office, vehicles and prep areas).
- Conduct weekly inventory checks of outreach supplies and communicate replenishment needs to the Leadership Team in a timely manner.
6. Other duties as assigned
OCCUPATIONAL HEALTH & SAFETY:
All employees are responsible for carrying out work in a way that does not adversely affect their own health and safety and that of others. All employees are expected to learn, understand, and adhere to the Busby Centre’s health and safety policies and procedures and keep up to date on any changes, particularly as it relates to new/emerging information related to infection prevention and control.
QUALIFICATIONS:
- Post-secondary education in Social Work, Social Service Work, Psychology, or a related field.
- 2+ years of experience in street outreach, case management, or working with marginalized populations.
- Knowledge of homelessness, poverty, mental health, substance use, and systemic barriers.
- Lived experience of homelessness and/or experience working with homeless and/or marginalized individuals is an asset.
- Familiarity with Housing First, harm reduction, and trauma-informed practices.
- Excellent interpersonal, verbal, and written communication skills and ability to set effective personal boundaries with both clients and co-workers.
- Certification or ability to become certified as soon as possible in NVCI (Non-Violent Crisis Intervention) and Level C First Aid and CPR, including working knowledge of AED, is required.
- Demonstrated competence to work with challenging behaviour and stressful situations and possess effective conflict and crisis management skills.
- Proven ability to problem-solve, multi-task and work independently in non-traditional settings (e.g., streets, encampments, vehicles).
- Valid driver’s license and access to a reliable vehicle required.
- Vulnerable Sector Screening (Police Check) is required.
WAGE: $26 per hour (negotiable upon experience)
SCHEDULE/WORKING CONDITIONS:
Contract, starting as soon as possible. The available positions may entail:
- Field-based role with frequent travel across urban and rural Simcoe County.
- Flexible hours, including evenings or weekends between the hours of 6am-11pm
- Exposure to adverse weather and high-stress situations.
- Long period of walking, standing, kneeling, sitting
- Walking in all terrain conditions
