Active Living Coordinator - Permanent, Full-time (35 hours/week)
The Alzheimer Society of Durham Region (ASDR) is a community support service organization that provides programs and services to people living with dementia and their care partners in Durham Region. We work closely with other service providers to ensure quality care and support for our clients to achieve our mission to improve the quality of life of people living with Alzheimer’s disease or a related dementia and their care partners.
We employ administrative and fund development staff, educators, social workers, social service workers, nurses, occupational therapists, recreation staff, and volunteers who work together as a team to meet the needs of people living with dementia and their care partners.

Under the direction of the Senior Manager, Education, Active Living & Wellness (Senior Manager), the Active Living Coordinator will be responsible for the facilitation, coordination, and expansion of the Active Living Programs, including Adult Day Program, Minds in Motion ® and The Social, as well as additional social and recreational programs as required. The Active Living Coordinator will work directly with clients delivering the programs, provide administration of program delivery and will also liaise with external partners, organizations, volunteers, and care partners.
What you will be doing
Service Delivery Responsibilities
- Conduct initial assessments to determine program eligibility and admission.
- Develop, implement, coordinate, and oversee ASDR Active Living programs designed to meet pre-established goals and objectives.
- Deliver social and recreation programming with an emphasis on co-design methodology, client-centered care, activation activities to address the physical, social, and emotional needs of individuals livings with dementia and care partners
- Responsible for the creation, implementation and updating of individual client care plans; monitoring client progress as it relates to the care plan and potential discharge/transition and communication with client care partners
- Develop and maintain community partnerships for the delivery of recreational & social programming.
- Support the Senior Manager in expanding Active Living Programs into the community and/or Partner Organizations
- Participate in the recruitment and onboarding of volunteers and students, including training, supervising, and supporting the delivery of the programs.
- Participate in data collection, monitor statistics to meet funder targets, and statistical reporting & program evaluation
- Work with the Senior Manager to develop program planning and delivery of social/recreation programs across Durham Region.
- Support marketing and promotion through collaboration with the Senior Manager and Communications Department
- Provide coverage for team across all Active Living Programs as assigned.
Administrative\ Organizational Responsibilities
- Complete and maintain case documentation and service delivery statistics in accordance with direct service policy requirements including attendance records, progress notes, waitlist management, client activity profiles and activity plans.
- Support the administration of daily duties including accurate attendance, ordering of supplies and groceries/catering, updating daily client information sheets i.e. transportation lists.
- Participate in agency related meetings, providing follow up action and involvement as designated, working collaboratively with colleagues in a positive team centered approach.
- Represent the Society on community and provincial committees as assigned.
- Participation and involvement in key agency annual fundraising events as required.

- Perform other duties consistent with the job classification, as required.
What you will bring along
Education:
- Post-secondary education in Health/Fitness Promotion, Gerontology, Activation/Recreation, or related degree with a comprehensive knowledge and understanding of Alzheimer’s disease and related dementias, Recreation and Activation Programming.
- Fitness Instructor/Personal Trainer or willingness to obtain.
Experience:
- Minimum 2 years of experience delivering recreational programs for adults with cognitive concerns, Day Program or senior fitness experience.
- Proven skills in therapeutic social and recreation programming
- Experience in working with families and care partners of persons living with Dementia or a related neuro-degenerative disease is preferred
- Demonstrated awareness of crisis de-escalation, behaviour management and intervention/adaptation techniques
- Experience administering psychosocial assessments (ex. Rai-CHA) is an asset
- Experience recruiting, interviewing, and managing multiple volunteers at once.
- Experience working in a not-for-profit environment preferred.
- Experience working with community services sector an asset
- Proficient computer skills in Microsoft Office and AlayaCare is an asset
Other Knowledge, Skills, Abilities or Certifications:
- Current First Aid/CPR Certification
- Certification in U-First! an asset
- Certification in Gentle Persuasive Approach, Montessori Methods, Crisis Prevention Training an asset
- Seniors’ Fitness Instructor Course (SFIC) an asset
- Dementia Inclusive Choices for Exercise (DICE) an asset
- Food Handler’s Certificate an asset
- Knowledge of and demonstrated experience working with persons living with a dementia and their care partners.
- Excellent interpersonal, communication, and documentation skills with the ability to work well with clients, families, all levels of staff, community partners and the public.
- Ability to manage multiple projects in a high-paced environment.
- Demonstrated ability to work both independently and collaboratively with all team members.
- Excellent computer skills including Word, PowerPoint, Excel, databases and competency in information and communication technology.
- Knowledge of and comfort using online communication platforms (i.e. Zoom, MSTeams)
- Demonstrated pattern of good attendance
- Ability to work occasional evening/weekend hours.
- Must demonstrate a commitment to customer service when dealing with clients, colleagues, volunteers, contractors, program facilitators and other members of the community and public.
- Current Ontario driver’s license.
Travel Requirements
- Successful candidates must be prepared to travel throughout Durham Region
- Must have insured vehicle available for daily use.
Physical Demands
- Expectation that there may be the need to assist participants with light personal care, on an infrequent basis. Limited to non-weight bearing assistance and verbal queuing.
- Physical activity may be required in various recreation programs. May require standing/sitting for long periods of time. Some heavier objects (such as fitness weights/furniture) may need to be moved or re-organized.
