Full job description

Wigwamen Incorporated is a long-established Indigenous non-profit housing provider serving the Toronto community since 1972. We are seeking a compassionate, organized, and client-focused individual to fill a 12-month full-time contract position as a Follow-up Support Worker. The Follow-up Support Worker will provide housing case management supports to Indigenous clients with moderate needs who have been homeless and recently housed. Flexibility to work some evenings and weekends will be required.

Why This Role Matters

Stable housing is often the foundation for improved health, well-being, and community connection. The Follow-up Support Worker helps Indigenous clients maintain housing, build independence, and access the services and supports they need to achieve long-term housing stability. Through a combination of outreach, advocacy, case management, and practical support, this role plays an important part in helping individuals successfully transition from homelessness to permanent housing.

Key Responsibilities

Client Support:

Conduct outreach and engagement activities with Indigenous individuals who are homeless or at risk of homelessness
Complete client assessments and identify support needs
Provide supportive counselling, crisis intervention, and housing stabilization supports
Conduct regular home visits; actively involve the client in planning, goal setting and decision-making
Provide responsive, culturally relevant, and client-centred services to help establish connections to community-based supports, services and programs
Actively participate in eviction prevention activities, including liaising with landlords to ensure rent is paid, and if needed, facilitate conflict and landlord mediation to encourage housing stabilization
Provide rehousing supports and assistance to clients where a tenancy cannot be sustained
Assist clients in moving towards achieving long-term housing and income stability, which may include advocating on behalf of clients with landlords, OW, ODSP, and other services
Provide life skills training in areas including landlord and tenant rights and responsibilities, budgeting, housekeeping, pet care, addiction treatment, mental health care, ID and document replacement services, volunteer opportunities, job placement, and job training etc.

Project Administration:

Develop and monitor an individualized case management plan for clients that focuses on setting and monitoring goals (both short and long-term), including discharge plans, crisis plans and mental health and addiction prevention plans (where applicable)
Keep detailed client records as set out in the overall project plan; submit monthly reports in accordance with Program requirements
Monitor the effectiveness of supports; recommend changes to the types of support offered
Other duties as reasonably required by Wigwamen Incorporated

Qualifications

Knowledge of, and experience working with, the urban Indigenous community
Familiarity with the “Housing First” approach
Minimum two years’ experience in a counselling role
Comprehensive knowledge of housing issues, advocacy, and support services
Ability to effectively navigate and assess community resources and supports
Excellent interpersonal, verbal communication, report writing and facilitation skills
Proficiency with Microsoft Office applications, email, and online case management or information systems
Experience working in a small team, within vulnerable and marginalized sectors and with clients with mental health issues and/or addictions
Working knowledge of an Indigenous and/or other language would be an asset
A valid driver's license and access to a vehicle would be an asset

Compensation & Benefits

This is a full-time, 12-month contract position, working 35 hours per week, at a rate of $31.59 per hour. A comprehensive benefits package is available following three months of employment.

There is potential for the contract to be renewed based on program needs and funding availability.
